Abstract
Zusammenfassung.Ziel: Der Krampfanfall stellt eine Komplikation des Alkoholentzugs dar. Die Studie vergleicht zwei Medikationsschemata bezüglich der Krampfanfallrate im stationären Alkoholentzug.
Methodik: Es wurde ein retrospektives, naturalistisches Herangehen gewählt. Patienten, die im Zentrum für Psychiatrie in Winnenden in der Klinik für Suchttherapie und Entwöhnung im Jahr 2009 und 2010 sowie von 2012 bis 2015 zum Zweck des qualifizierten Alkoholentzugs behandelt wurden, wurden hinsichtlich der Krampfanfallrate untersucht. Es kamen in den Zeiträumen zwei unterschiedliche Medikationsschemata im Entzug (altes Schema [AS]: starres Schema mit Clomethiazol und Oxcarbazepin; neues Schema [NS]: flexibles Schema mit Clomethiazol) zum Einsatz.
Ergebnisse: Es wurden N = 10 836 Fälle (AS: n = 2675 bzw. NS: n = 8161) untersucht. Patienten, die mit dem AS behandelt wurden, waren signifikant älter und hatten eine längere Verweildauer. Die Geschlechterverteilung war nicht unterschiedlich. Die Krampfanfallrate, nicht jedoch die Zeit bis zum Eintritt eines Krampfanfalls, war auch unter Berücksichtigung von Alter und Verweildauer signifikant geringer (p < .0001) unter Anwendung des NS (0.527 %) verglichen mit dem AS (1.607 %).
Schlussfolgerungen: Trotz methodischer Einschränkungen, die sich aus dem nichtkontrollierten Ansatz ergeben, zeigt sich unter Anwendung eines risikostratifizierten Schemas mit symptomgetriggerter Eindosierungs- und starrer Abdosierphase eine signifikant geringere Krampfanfallrate.
Abstract.Objective: Seizure constitutes a severe complication of alcohol withdrawal. Diverse strategies for treating alcohol withdrawal exist. These are recommended by the S3-guidelines for alcohol-related disorders. Aim of the study is to analyse whether two detoxification sched- ules differ concerning the seizure frequency.
Methods: We have chosen a naturalistic, retrospective study design. Patients treated in Psychiatric Hospital in Winnenden in 2009, 2010 and from 2012 to 2015 for alcohol detoxification were analysed regarding seizure frequency. Two different detoxification schedules were used.
Results: Overall, N = 10 836 cases could be included (treatment as usual [TAU]: n = 2675 compared to new schedule [NS]: n = 8161). Patients treated with TAU were significantly older and had a longer hospital stay. No differences were found concerning distribution between the sexes. Seizure frequency, but not time to seizure, was significantly lower (p < .0001) among NS-patients (0.527 %) compared to TAU-patients (1.607 %).
Conclusions: Despite methodological limitations arising from the uncontrolled study approach, a significantly lower seizure frequency could be achieved using a risk-stratified detoxification schedule with a symptom-triggered titration phase and a fix tapering phase.
