Kognition und Gedächtnis bei Patienten mit schlafbezogenen Atmungsstörungen

Verlaufsuntersuchung über eine dreimonatigen nCPAP-Therapie

Neben Tagesschläfrigkeit sollen Patienten mit schlafbezogenen Atmungsstörungen (SBAS) ohne nCPAP-Therapie (nasal Continuous Positive Airway Pressure) auch kognitive und mnestische Einbussen zeigen, vorrangig in exekutiven Funktionen. Wir vergleichen neuropsychologische Leistungen von 20 Patienten mit leichten und schweren SBAS und betrachten deren Verlauf nach drei Monaten der laufenden nCPAP-Therapie. Hierzu wurden polysomnographische und neuropsychologische Daten von männlichen Patienten mit SBAS vor Beginn und nach dreimonatiger Gerätetherapie erhoben. Auch wenn die erfassten Leistungen vor Therapiebeginn im noch durchschnittlichen Bereich lagen, konnte durch die Ventilationsbehandlung eine Verbesserung der Leistungen insbesondere in exekutiven und mnestischen Funktionen erreicht werden. Es zeigen sich Unterschiede in den verbesserten Bereichen zwischen den leicht und schwer schlafgestörten Patienten. Abschliessend wird dieser Unterschied hinsichtlich störungsspezifischer Parameter diskutiert.


Cognitive and mnestic functions in patients with sleep related breathing disorders – Follow-up after three months of nCPAP-therapy

Besides daytime drowsiness patients with sleep-related breathing disorders (SRBD) without nCPAP-therapy (nasal Continuous Positive Airway Pressure) have been reported to reveal cognitive and mnestic deficits, mainly in executive functions. We compare neuropsychological data of 20 male patients with light and severe SRBD tested before and after three months of their ongoing nCPAP-therapy. Though revealing just average performance in most of the assessed neuropsychological tests, all patients as a consequence of nCPAP-therapy improved significantly with respect to certain cognitive, especially executive functions, and mnestic abilities. Differences between the two patient-groups with regard to the improved neuropsychological functions were noted and are discussed in the light of disorder-specific parameters.
