Abstract
Zusammenfassung. Drogenkonsumierende sind mit einer Prävalenz von 56 % die grösste Hepatitis-C-Risikogruppe in der Schweiz. Ihre ungenügende Versorgungssituation stellt ein ernsthaftes Problem für die öffentliche Gesundheit dar, einerseits durch die Weiterverbreitung des Virus und anderseits durch die Folgeerkrankungen der Infektion mit den entsprechenden Kosten. Es sind neue Ansätze gefragt in der Hepatitis-C-Versorgung bei Drogenkonsumierenden. Eine Suchterkrankung und auch Hepatitis C sind chronische Leiden, die häufig von weiteren chronischen Krankheiten begleitet werden. Sie bedürfen einer integrierten multidisziplinären Versorgung. Mit einer Einbindung der Hepatitis-C-Versorgung in das medizinische Grundversorgungssetting können bislang unerreichte Patientengruppen behandelt werden. Die aktuelle Entwicklung in der HCV-Therapie, weg von anspruchsvollen interferonbasierten Behandlungsschemata und hin zu einfachen Kombinationstherapien, unterstützt die Versorgung dieser Risikogruppe.
Abstract. With a 56 % prevalence of hepatitis C among people who use drugs, they are the highest risk group in Switzerland. Their insufficient access to medical care has become a huge public health issue; the virus is spreading further and is causing secondary illnesses, which costs money. A new approach is needed in hepatitis care among drug users. Addiction and hepatitis C are both chronic illnesses, which are often accompanied by other chronic ailments. They require a multidisciplinary integrated approach. If hepatitis C care is integrated into doctors’ surgeries, patients can be reached who could not be treated so far. The new interferon-free hepatitis C treatments help provide this risk group with the necessary medical care.
Résumé. En Suisse, les personnes consommant de la drogue appartiennent avec une prévalence de 56 % au groupe à risque le plus élevé de contracter l’hépatite C. Dû à leur accès insuffisant aux soins, cela représente un problème pour la santé publique. D’une part, à cause du danger de propagation du virus et d’autre part en raison des maladies provoquées par celui-ci ainsi que les coûts que cela engendre. Ceci requiert de nouvelles approches pour la prise en charge de l’hépatite C chez les consommateurs de drogues, car la dépendance et l’hépatite C sont des souffrances chroniques qui souvent vont de pair avec d’autres maladies chroniques et qui nécessitent une assistance multidisciplinaire intégrée. L’intégration de la prise en charge de l’hépatite C dans la prise en charge de base aurait le potentiel de donner un accès aux soins à des groupes de patients jusqu’alors privés de suivi médical. Le développement actuel des traitements de l’hépatite C s’éloigne des schémas thérapeutiques exigeants à base d’interférons, augmentera le potentiel de traitement de les malades à haut risque.
