Abstract
Zusammenfassung. Chronisch kranke und multimorbide Menschen erfordern neue Versorgungsmodelle, in denen die Zusammenarbeit der Berufsgruppen neu definiert wird. Während sich interprofessionelle Modelle international bewähren, bestehen in der Schweiz erst wenige solche Versorgungsteams. Insbesondere ist wenig bekannt über die Ansicht von Hausärzten zu neuen Versorgungsmodellen mit Advanced Practice Nurses (APNs). Deshalb untersuchten wir, wie Hausärzte mögliche Zusammenarbeitsformen mit APNs in der Grundversorgung beschreiben. Wir führten Fokusgruppeninterviews mit Hausärzten durch. Vorgängig explorierte ein Expertengremium Themen der interprofessionellen Zusammenarbeit. Die Interviews wurden mittels qualitativer Inhaltsanalyse analysiert. Die Analyse ergab, dass den Ärzten die Beibehaltung der heutigen Versorgungsqualität wichtig ist. Sie stehen einer Zusammenarbeit mit APNs offen gegenüber, wissen aber wenig über deren Kompetenzen. Erschwerend wirkt sich zudem aus, dass die Finanzierung noch nicht geregelt ist.
Abstract. Multimorbid and chronically ill patients require models of care for which the collaboration of all involved professionals must be redefined. Inter-professional teams are successful in certain countries. However, little is known about the views of Swiss Primary Care Physicians (PCPs) on models of care involving advanced practice nurses (APNs). Therefore, we explored PCPs’ views on potential models of collaboration. Focus groups were conducted with PCPs. In advance, topics of inter-professional collaboration were explored by a group of experts. The interviews were analysed by qualitative content analysis. The results show that maintaining the quality of care is important to PCPs. They are willing to collaborate, but PCPs know little about an APN’s expertise. Furthermore, the situation is made difficult by the fact the financing of non-medical services is not yet regulated.
Résumé. Les exigences concernant les soins primaires évoluent avec les changements démographiques. En Suisse, rares sont les équipes de collaboration interprofessionnelle en comparaison avec l’international. Aujourd’hui, la position des médecins sur ces modèles est peu connue. Nous avons donc étudié comment ils décrivent les formes possibles de coopération. Nous avons élaboré, avec un groupe d’experts, des thèmes sur la collaboration interprofessionnelle. Puis nous avons mené des groupes de réflexion avec les médecins, analysés suivant une analyse de contenu qualitative. Les résultats montrent que les médecins sont soucieux du maintien de la qualité actuelle des soins, ouverts à une pratique collaborative; cependant peu connaissent les compétences des infirmières praticiennes de niveau avancé. Le financement doit être abordé pour favoriser la mise en place de ces pratiques.
