Abstract
Zusammenfassung. Vor dem Start unseres Projekts gab es im St. Francis Referral Hospital, Ifakara, Tansania, wie auch in den meisten Spitälern im südlichen Afrika keine Notfallstation. Durch eine Ausbildung in Notfallmedizin und Ultraschall, die Organisation eines Dreischichtbetriebes und einer Triage war die neu geschaffene Notfallstation weniger als sechs Monate nach Beginn des Projekts voll funktionstüchtig. Einige wenige Geräte (zwei Überwachungsmonitore, EKG-Gerät, Thoraxdrainagesystem, Vernebler, Defibrillator, Ultraschallgeräte) mussten angeschafft werden oder wurden gespendet. Derzeit werden täglich 80–100 Patienten betreut, etwa 20 % der Patienten sind schwer krank. Neben Anamnese und klinischer Untersuchung ist der Ultraschall das wichtigste diagnostische Instrument. Die klinische Arbeit steht im Mittelpunkt. In einer Studie werden mittels «Focused Assessment with Sonography for HIV-Associated Tuberculosis (FASH)» Patienten mit Verdacht auf Tuberkulose untersucht, mit Echokardiografie ermitteln wir die Häufigkeit von Herzkrankheiten.
Abstract. Before the start of our project there was no emergency department at the St. Francis Referral Hospital, Ifakara, Tanzania, as is the case with most hospitals in southern Africa. With an education in emergency medicine and ultrasound, the creation of a three-shift operation and a triage, the newly created emergency department was fully functional within less than six months. A few devices (two surveillance monitors, ECG device, thoracic drainage system, nebulizer, defibrillator, and ultrasound machines) had to be purchased or were donated. Currently, 80–100 patients are being cared for daily, with about 20 % of patients being seriously ill. In addition to history taking and clinical examination, ultrasound is the most important diagnostic tool. Clinical work is the focus. In a study, patients with suspected tuberculosis are examined using focused assessment with sonography for HIV-associated tuberculosis (FASH), and echocardiography is used to determine the frequency of heart disease.

Résumé: Avant le début de notre projet il n’y avait pas de station de soins d’urgence à l’Hôpital de référence St-François, à Ifakara en Tanzanie, comme c’est le cas dans la plupart des hôpitaux en Afrique du sud. Grâce à une formation en médecine et ultrasonographie d’urgence, la mise en place de trois équipes et d’un triage, la station de soins d’urgence nouvellement créée a été fonctionnelle en moins de six mois. Il a été nécessaire d’acheter quelques appareils (deux systèmes de surveillance, un appareil ECG, un système de drainage thoracique, un nébuliseur, un défibrillateur et des appareils à ultrasons), certains ont été donnés. Actuellement, 80 à 100 malades sont pris en charge chaque jour, le 20 % d’entre eux étant sérieusement malades. En plus de l’anamnèse et de l’examen clinique, l’ultrasonographie est l’outil diagnostique le plus important. Le travail clinique est le centre d’intérêt. Dans une étude, les patients chez qui une tuberculose est suspectée sont examinés par échographie selon l’approche utilisée pour évaluer la tuberculose associée au HIV (Focused Assessment with Sonography for HIV-Associated Tuberculosis), et l’échocardiographie est utilisée pour déterminer la fréquence des maladies cardiaques.
