Insomnie – ein Symptom mit multifaktorieller Genese
Diagnostik primärer und sekundärer Insomnien
Abstract
Zusammenfassung. Insomnien sind ein klinisch relevantes Symptom und betreffen bis zu 30 % der Allgemeinbevölkerung. 80 % der Betroffenen leiden bereits chronisch an der Schlafstörung, bevor eine suffiziente Diagnostik oder Therapie eingeleitet wird. Die Diagnostik primärer und sekundärer Insomnien kann durch schlafspezifische Anamnesefragen und die hier dargestellte Stufendiagnostik hausärztlich erfolgen, die Früherkennung verbessern und eine fachärztliche Weiterbehandlung rechtzeitig initiieren. Spezielle neurologisch bedingte Schlafstörungen nach Schlaganfall, Schädel-Hirn-Trauma und bei neurodegenerativen Erkrankungen werden erläutert, da sie häufig unzureichend erkannt und behandelt werden. Zusammenfassend kann die hausärztliche Früherkennung primärer und sekundärer Insomnien die Chronifizierung sowie das Risiko für zerebrovaskuläre, metabolische, psychiatrische oder andere Folgeerkrankungen reduzieren und die Lebensqualität der Betroffenen verbessern.
Abstract. Insomnia is a relevant symptom with a multifactorial genesis and affects up to 30 % of the general population. 80 % of patients suffer from chronic insomnia before adequate diagnosis or therapy is initiated. Sleep-specific anamnesis and diagnostic algorithms can facilitate differential diagnosis of primary and secondary insomnia by general practitioners. Thereby, early specialist treatment can be initiated. Furthermore, this article highlights some sleep disturbances associated with neurological diseases like stroke, traumatic brain injury and neurodegenerative disorders. These patients are often seriously affected by disease-specific sleeping disorders which are often insufficiently diagnosed and treated. In conclusion, early diagnosis of primary and secondary insomnia can help avoid chronification, reduce cerebrovascular, metabolic, psychiatric or other secondary diseases and improve health-related quality of life.

Résumé: L’insomnie est un symptôme relevant d’origine multifactorielle qui touche jusqu’à 30 % de la population générale. 80 % des malades souffrent d’une insomnie chronique avant qu’un diagnostic adéquat soit posé ou qu’un traitement soit initié. Une anamnèse spécifique portant sur le sommeil et des algorithmes diagnostiques peut faciliter le diagnostic différentiel par les médecins généralistes entre insomnie primaire et secondaire. Un traitement précoce par un spécialiste peut alors être initié. Par ailleurs, cet article met en exergue quelques troubles du sommeil associés à des affections neurologiques comme l’accident vasculaire cérébral, le traumatisme crânien et des affections neurodégénératives. Ces malades sont souvent sévèrement affectés par les troubles du sommeil spécifiques à leur maladie, troubles qui sont souvent insuffisamment diagnostiqués et traités. En conclusion, un diagnostic précoce de l’insomnie primaire et secondaire peut aider à éviter que le trouble devienne chronique, diminuer des affections secondaires comme les accidents vasculaires cérébraux, les affections métaboliques et psychiatriques ainsi que d’autres maladies et améliorer donc la qualité de vie en relation à la santé.
