Abstract
Zusammenfassung. Studien zur Patientenbeteiligung in der medizinischen Versorgung deuten darauf hin, dass eine intensive Patientenbeteiligung die Gesundheit verbessert und die Vorsorgungszufriedenheit erhöht. Insbesondere bei Behandlungsentscheidungen, die sowohl Schaden als auch Nutzen verursachen, sind gemeinsame Entscheidungen von Patienten und Ärzten entscheidend um den Patienten auf den Behandlungsplan vorbereitet und ihn dafür zu motivieren. Allerdings sind Instrumente, die den Patienten in den klinischen Versorgungsprozess und gemeinsamen Entscheidungsfindung einbeziehen, nicht weit verbreitet. Wir entwickelten ein klinisches Patientendokument für Patienten mit autosomal-dominanter polyzystischer Nierenerkrankung (ADPKD), für die kürzlich eine neue Therapie – Tolvaptan, das die Progression verlangsamt, aber bemerkenswerte Nebenwirkungen hat – zur Verfügung steht. Untersuchungen vor und nach der Entwicklung zeigten, dass ADPKD-Patienten eine Anzeige individueller Schweregradindikatoren mit Benchmarks für vergleichbare Patienten wünschen. Die Kommunikation wird erleichtert und das Vertrauen der Patienten in die gemeinsam gefällten Behandlungsentscheidungen verbessert. Ein solcher Patientenbericht ist leicht auf andere Krankheitsbilder übertragbar und könnte als ein Aspekt eines Patienteninterventionsmodells in klinischen Zentren dienen.
Abstract. Studies of patient participation in medical care suggest that a higher degree of patient involvement can lead to improved health outcomes and more satisfaction with care. For treatment choices in particular, which often involve harms as well as benefits, shared patient-physician treatment decisions are critical to assuring patients are prepared for and committed to the treatment plan. However, tools to facilitate patient inclusion in the clinical care process and shared decision making are not wide-spread or well-tested. Here we developed a clinical patient reporting tool for an autosomal dominant polycystic kidney disease (ADPKD) patient population for whom a new therapy – Tolvaptan, which slows progression but has notable side effects – has recently become available. Pre-development and post-development surveys indicated that a display of relevant individual disease severity indicators over time with benchmarks to similar patients is desired by ADPKD patients and can facilitate communication, improving patient confidence in sharing treatment decisions. Such a patient report is easily transferable to other disease settings and could serve as one aspect of a patient engagement model in clinical centers.
Résumé. Les études concernant la participation des malades aux soins médicaux suggèrent qu’une participation accrue du patient peut améliorer le devenir sur le plan de la santé et donner plus de satisfaction sur le plan du traitement. Pour le choix des traitements, qui sont souvent à la fois délétères et bénéfiques, les décisions partagées par le médecin et le malade sont critiques pour s’assurer que les malades sont prêts à accepter et à appliquer le plan de traitement. Néanmoins, les moyens de faciliter la participation du patient dans le choix de la prise en charge ne sont pas largement diffusés et demeurent mal testés. Nous avons développé un moyen d’impliquer le malade présentant une polykystose rénale à caractère autosomique dominant (ADPKD), une maladie pour laquelle existe un nouveau médicament, le tovalpan, qui ralentit la progression de la maladie, mais avec des effets secondaires non négligeables. Les enquêtes de surveillance pré- et post-développement du médicament ont mis en évidence qu’une présentation des indicateurs de sévérité sur le plan individuel avec des points de comparaison avec d’autres patients similaires est souhaitée par les malades et peut faciliter la communication et ainsi améliorer la confiance dans le partage décisionnel du traitement. Un tel outil de présentation peut facilement être transposé à d’autres affections médicales et pourrait représenter un modèle de participation aux soins du malade dans les centres cliniques.
