Kolonkarzinom – Prävention 2008

Eine sinnvolle und effektive Prävention des Kolonkarzinoms ist heute möglich. Dabei sind verschiedene Faktoren von Bedeutung. In der Primärprävention gibt es positive Daten für die Empfehlung zur regelmässigen körperlichen Bewegung, zur Gewichtsreduktion bei einem BMI > 25 kg/m² sowie zum Verzicht auf Nikotin. Der Alkoholkonsum sollte gering gehalten werden. Der Einfluss der Ernährung scheint insgesamt gering zu sein bzw. die Datenlage ist uneinheitlich. Empfohlen wird eine ballaststoffreiche Kost. Der Verzehr von rotem bzw. verarbeitetem Fleisch gilt als nachteilig. Nahrungsergänzungsmittel wie Vitamine oder Antioxidantien sind gänzlich ungeeignet, das Risiko, an einem kolorektalen Karzinom zu erkranken, zu reduzieren. Der Einsatz von Medikamenten wie ASS, NSAR oder Statine in der Primärprävention kann ebenfalls nicht empfohlen werden. Der Haemoccult-Test ist derzeit noch Standard bei den nicht-invasiven Stuhltests. Der neuere immunochemische Test auf humanes Hämoglobin weist jedoch bessere Ergebnisse hinsichtlich Sensitivität und Spezifität auf. Der TuM2-PK-Test ist eine weitere Alternative, die dem immunochemischen Test jedoch unterlegen zu sein scheint. Die Koloskopie muss als Goldstandard in der Prävention des kolorektalen Karzinoms angesehen werden. Ziel aller Bemühungen muss es sein, die Akzeptanz in der Bevölkerung deutlich zu erhöhen.

Une prévention judicieuse et efficace du carcinome du colon est possible à l’heure actuelle. Différents facteurs sont importants dans ce contexte. En prévention primaire, l’exercice physique régulier, la réduction du poids jusqu’à un BMI >25 kg/m² et l’abstinence de tabac donnent des résultats positifs. La consommation d’alcool devrait rester modérée. L’influence de la nutrition semble globalement faible respectivement les données à disposition sont en partie discordantes. On recommande une alimentation riche en fibres. La consommation de viandes rouges respectivement de viandes traitées est considérée comme défavorable. Les compléments nutritionnels tels que les vitamines ou les antioxydants sont totalement inapropriés pour diminuer le risque de cancer colorectal. Des médicaments tels que l’aspirine, les anti-inflammatoires non-stéroïdiens ou les statines ne peuvent également pas être recommandés en prévention primaire. Le test Hemoccult reste actuellement encore le test standard non-invasif. Les nouveaux tests immunochimiques de l’hémoglobine humaine démontrent cependant de meilleurs résultats de sensibilité et spécificité. Le test TuM2-PK est une alternative, il semble cependant inférieur au test immunochimique. La colonoscopie doit être considérée comme le «gold standard» de la prévention du carcinome colorectal. Le but de tous les efforts est d’augmenter son acceptation dans la population.

Today effective prevention of colorectal cancer can be done. For primary prevention there are good data for the recommendation of regular physical activity and body weight reduction if BMI is greater than 25 kg/m². Furthermore colorectal cancer risk correlates with alcohol intake and the harmful role of tobacco is well documented. But the influence of nutrition seems to be small. The results of a lot of studies are inconsistent. Diets high in fiber are recommended. The intake of red or processed meat seems to be associated with a higher risk for colorectal cancer. With the additive intake of vitamins and antioxidants the risk of colorectal cancer cannot be reduced. At present the use of drugs like aspirin, NSAD or statins cannot be recommended for primary prevention. The fecal occult blood test (FOBT) is the standard of the non-invasive screening methods. But regarding sensitivity and specifity there are better results for the new immunochemical fecal occult blood tests. The TuM2-PK test is an another screening method, but seems to be inferior to the immunochemical tests. Colonoscopy is the gold standard in the prevention of colorectal cancer. But it is important to improve the acceptance of this screening method in general population.
