Verbessern verhaltensmedizinische Behandlungsstrategien die Lebensqualität bei Kindern mit chronischen Kopfschmerzen?

Zusammenfassung.Theoretischer Hintergrund: Kinder, die unter chronischen Kopfschmerzen leiden, erleben eine reduzierte Lebensqualität, die sie vielfach im Alltag beeinträchtigen. In der vorliegenden Studie wurde die Wirkung eines multimodalen Gruppenschulungsprogramms zur Behandlung kindlicher Kopfschmerzen (MIPAS-Family) und eines Biofeedbacktraining im Hinblick auf die Veränderung der erlebten Einschränkungen untersucht. Patienten und Methoden: Insgesamt 34 Kopfschmerzkinder im Alter zwischen 7 und 16 Jahren (MIPAS-Family-Gruppe, N = 19; Biofeedbackgruppe, N = 15) nahmen an der Studie teil. Alle Kinder wurden entsprechend den Kriterien der International Headache Society (IHS) klassifiziert. Zur Bestimmung der Lebensqualität wurde den Kindern der KINDL® und den Eltern die Impact of Family Scale sowie der Diagnostische Elternfragebogen (DEF) im Prä-Post-Vergleich vorgelegt. Ergebnisse: Die Kinder waren nach beiden Behandlungen weniger in den Bereichen Schule, Hausaufgaben und Freizeit durch die Kopfschmerzen beeinträchtigt. Die Verbesserungen der Lebensqualität sind vor allem in der Nachbeobachtung nach einem Jahr festzustellen. Schlussfolgerungen: Es konnte gezeigt werden, dass ein multimodales Training in der Gruppe (MIPAS-Family) ebenso effektiv ist wie ein aufwendigeres und kostenintensiveres Biofeedbacktraining in Einzelbehandlung.


Do behavioral medicine approaches have an effect on the quality of life and everyday competence of children suffering from chronic headaches?

Abstract.Background: Children suffering from chronic headaches experience a significant decrease in quality of life which greatly affects their everyday life. The present study compared the impact of a new multimodal behavioral treatment (MIPAS-Family) to a biofeedback treatment on measures of quality of life for children with chronic headaches. Patients and methods: Thirty-four recurrent headache sufferers, ranging from 7 to 16 years, were randomly assigned to the MIPAS-Family (N = 19) or the biofeedback (N = 15) condition. All patients were diagnosed according to the criteria of the International Headache Society (IHS). The children and their parents completed the following questionnaires prior to and following treatment as well as one year later: the KINDL® (which assess quality of life), the Impact of Family Scale, and a German parental questionnaire which assess child behavior. Results: Both groups showed significant improvements in the domains assessed by the KINDL–school, homework, and leisure-time activity. These effects endured for one year after the completion of treatment. Conclusions: MIPAS-Family and biofeedback both lead to significant improvements in quality of life in children experiencing recurrent headaches.
