Medikamentöse Varikosetherapie aus der Perspektive experimenteller Modelle
Abstract
Zusammenfassung. Variköse Veränderungen der Venenwand entstehen vorrangig in den unteren Extremitäten und sind häufig mit venöser Insuffizienz vergesellschaftet. Obgleich für einen grossen Teil der westlichen Bevölkerung eine gering- bis höhergradige Varikosis nachgewiesen werden kann, ist nur wenig über deren Entstehungsmechanismen bekannt. Experimentelle Tiermodelle haben in den vergangenen Jahren dazu beigetragen, Zielmoleküle und zelluläre Mechanismen zu identifizieren, die variköse Umbauprozesse steuern. So scheint die chronische Erhöhung der venösen Wandspannung eine entscheidende Determinante zu sein, um Signalkaskaden zu stimulieren, an deren Ende eine gesteigerte proteolytische und proliferative Aktivität venöser Gefässwandzellen steht. Die pharmakologische Hemmung von für diese Prozesse entscheidenden Schlüsselmolekülen stellt möglicherweise einen Weg dar, den Verlauf und die Ausprägung der Varikosis zu beeinflussen. Dieser Übersichtsartikel gibt einen kurzen Einblick in diese Thematik.
Abstract. Varicose remodeling of the venous wall primarily occurs in the lower extremities and is often associated with venous insufficiency. Although a large part of the western population shows various degrees of varicosis, little is known about the mechanisms driving their formation. In recent years, experimental animal models have spurred the identificatoin of target molecules and cellular mechanisms that control varicose remodeling processes. Thus, the chronic increase in venous wall tension appears to be a crucial determinant to stimulate signal cascades, culminating in increased proteolytic and proliferative activity of venous wall cells. The pharmacological inhibition of key molecules in these processes may provide a way to influence the course and severity of varicosis. This review article gives a brief insight into this topic.
Résumé. Les modifications variqueuses de la paroi veineuse se produisent principalement dans les membres inférieurs et sont souvent associées à une insuffisance veineuse. Bien qu’une grande partie de la population de l’Ouest présente un degré de varicose faible à élevé, on en sait peu sur les mécanismes de son développement. Ces dernières années, des modèles animaux expérimentaux ont contribué à l’identification de molécules cibles et de mécanismes cellulaires qui contrôlent les processus de remodelage variqueux. L’augmentation chronique de la tension de la paroi veineuse semble être un déterminant décisif pour stimuler les cascades de signaux qui conduisent à une activité protéolytique et proliférative accrue des cellules de la paroi veineuse. L’inhibition pharmacologique des molécules clés qui sont cruciales pour ces processus peut être un moyen d’influencer l’évolution et la gravité de la varicose. Cet article de synthèse donne un bref aperçu de ce sujet.
