Sekundäre Lues mit okulärem Befall und Schleimhautulzera

In unserer Arbeit berichten wir über einen Patienten, welcher sich mit einer schmerzlosen Läsion am Kinn beim Hausarzt präsentierte. Trotz diverser Therapieversuche mit Antibiotika und Steroiden kam es nicht zur Abheilung. Im Verlauf traten schmerzhafte Erosionen im Mund, perianal juckende Läsionen und ein Zentralskotom auf. Im Rahmen der Abklärungen konnte eine Lues II mit Augenbeteiligung (Makulaödem) und multiplen schmerzhaften enoralen Ulzerationen diagnostiziert werden. Die Hautläsion am Kinn wurde aufgrund des zeitlichen Zusammenhangs zwischen Exposition und Auftreten als Primäraffekt gedeutet. Wir beschreiben die Epidemiologie und diagnostische Massnahmen der Lues. Auf deren Manifestationsformen und ihr Wandel in der heutigen Zeit wird eingegangen, aktuelle Therapierichtlinien werden genannt.

In this case-report we describe a patient who was first seen with a painless lesion of the chin. Despite treatment with antibiotics and steroids it didn't heal. Later on, painful ulcerations of the mouth, perianal itching lesions and a central skotoma appeared. In the hospital-course a syphilitic infection with multiple oral ulcerations and occular infestation was diagnosed. The lesion on the chin was interpreted as the primary chancre as a time-correlation of its appearance and the time of exposition could be documented. The epidemiology, the diagnostic measures are described. We outline the manifestations of syphilis and their change in our time and discuss therapeutic regimes.

Ce travail présente le cas d'un patient qui consulte son médecin-traitant pour une lésion indolore au menton. Divers traitements par antibiotiques et stéroïdes restent sans succès. Par la suite, le patient présente des érosions douloureuses dans la bouche, des lésions préanales avec prurit et un scotome central. Les investigations complémentaires mettent en évidence une syphilis secondaire avec une atteinte oculaire (oedème maculaire) et des multiples ulcérations orales douloureuses. Les lésions cutanées au menton ont été considérées comme l'infection primaire en raison de la relation temporelle entre l'exposition et l'apparition des premiers symptômes. Nous décrivons l'épidémiologie et le traitement de la syphilis: Les formes de manifestation et leur évolution actuelle sont également présentées avec les dernières directives thérapeutiques.
