Intermittierende oder persistierende Rhinitis bei Kindern und Jugendlichen mit Asthma: «The Swiss LARA paediatrics survey»

Asthma und allergische Rhinitis sind chronisch entzündliche Erkrankungen der Atemwege, die oft gemeinsam auftreten. Das LARA-Programm (Link Allergic Rhinitis in Asthma) hatte zum Ziel, die aktuelle Komorbidität und Charakteristiken von Asthma (A), intermittierender oder persistierender Rhinitis (IPR) und durch den behandelnden Arzt definierter atopischer Dermatitis (AD) bei Kindern und Jugendlichen (6 bis 16 Jahre) mit Asthma in der Schweiz zu erfassen. Insgesamt erfassten 126 Hausärzte und Pädiater die diesbezüglichen Daten von 670 Asthmapatienten. Rund ein Drittel der Kinder mit Asthma war gut kontrolliert. Fast drei Viertel der Kinder mit Asthma litten gleichzeitig unter einer IPR. Bei vergleichbarem Verbrauch von inhalierten Kortikosteroiden (rund 90%) und Leukotrien-Rezeptor-Antagonisten (rund 50%) wiesen letztere signifikant weniger Symptome auf als diejenigen ohne IPR. Allerdings waren in der weniger gut kontrollierten Gruppe fast doppelt so viele Passivraucher. Die Prävalenz von AD war in beiden Gruppen vergleichbar. IPR und AD könnten eine wichtige Rolle als Risikofaktoren für die weitere Entwicklung des Asthmas spielen.

L'asthme et la rhinite allergique sont des maladies chroniques des voies respiratoires souvent associées. Le programme LARA avait pour objectif de documenter la comorbidité et les caractéristiques de l'asthme (A), de la rhinite intermittente ou persistante (RIP) et de la dermatite atopique (DA) telles que définies par le médecin-traitant chez les enfants et adolescents (6 à 16 ans) asthmatiques en Suisse. Au total, 126 médecins généralistes et pédiatres ont collecté les données de 670 patients asthmatiques. Environ un tiers des enfants asthmatiques était bien contrôlé. Près des trois quarts des enfants asthmatiques présentaient une RIP concomitante. Malgré une utilisation comparable de cortico-stéroïdes inhalés (environ 90%) et d'antagonistes des récepteurs aux leucotriènes (environ 50%), ces derniers présentaient significativement moins de symptômes. Par contre, il y avait presque deux fois plus de fumeurs passifs dans le groupe moins bien contrôlé. La prévalence de la DA était similaire dans les deux groupes. RIP et DA pourraient être des facteurs de risque importants dans l'évolution future de l'asthme.

Asthma and allergic rhinitis are chronic inflammatory airway diseases which often occur concomitantly. The objective of the LARA program was to identify the comorbidities and characteristics of asthma (A), intermittent or persistent rhinitis (IPR) and physician defined atopic dermatitis (AD) in 6- to 16-year old asthmatic Swiss children and adolescents. Overall, 126 general practitioners and paediatricians collected the data of 670 asthmatics. Approximately one third of the asthmatic children in Switzerland had well-controlled asthma. Almost two thirds of these asthmatics suffered from concomitant IPR. The latter presented with significantly less symptoms while the treatment rates with inhaled corticosteroids (approximately 90%) and leukotriene-receptorantagonists (approximately 50%) were comparable. However, there were almost twice as many passive smokers in the less well-controlled group. The prevalence of AD was similar in both groups. IPR and AD may play an important role as risk factors in the future development of asthma.
