Abstract
Zusammenfassung. Erwachsene in der Schweiz trinken durchschnittlich zwei Gläser Wein pro Tag. Alkoholmissbrauch und kognitive Störungen sind eng verknüpft, vor allem bei älteren Personen. Die klinische Untersuchung muss nach Anzeichen von Unterernährung suchen und eine neuropsychologische Untersuchung einschliessen, wenn der Mini-Mental-Status pathologisch ist. Bei Anzeichen einer Gayet-Wernicke-Krankheit muss eine intravenöse B1-Vitamin-Therapie zur prophylaktischen oder therapeutischen Behandlung eingeleitet werden. Kognitive Beeinträchtigung ist eine schlechte Prognose für die Behandlung von Alkoholabhängigkeit, sollte aber nicht dazu führen, die Behandlungsbemühungen aufzugeben.
Abstract. Adults in Switzerland drink an average of two glasses of wine per day. Alcohol abuse and cognitive disorders are closely linked, especially in elderly persons. The clinical examination must look for signs of malnutrition and include a neuropsychological examination when the Mini Mental State is pathological. In the presence of signs suggestive of a Gayet-Wernicke syndrom, an intravenous B1 vitamin therapy must be initiated for prophylactic or therapeutic treatment. Cognitive impairment is a factor of poor prognosis for the treatment of alcohol dependence, but should not lead to abandon treatment efforts.
Résumé. Les adultes en Suisse consomment en moyenne l’équivalent de deux verres de vin par jour. Abus d’alcool et troubles cognitifs sont fortement liés, particulièrement chez les personnes âgées. L’examen clinique doit rechercher des signes de dénutrition et comprendre un examen neuropsychologique, lorsque le mini mental state est pathologique. Une vitaminothérapie B1 intraveineuse doit être entreprise de manière prophylactique ou thérapeutique en fonction de la présence de signes évocateurs de syndrome de Gayet-Wernicke. Les troubles cognitifs constituent un facteur de mauvais pronostic pour le traitement de la dépendance à l’alcool mais ne doivent pas mener à y renoncer.
