Folienrauchen/Heroin inhalativ
Abstract
Zusammenfassung. Folienrauchen bezeichnet einen inhalativen Konsum von Heroin. Nach Erhitzung und Verflüssigung des Heroins durch Anwendung eines Feuerzeugs auf der Unterseite einer Aluminiumfolie wird mittels Strohhalm der dadurch entstehende Rauch inhaliert. Neben dem bekannten intravenösen (IV) Konsum ist das Folienrauchen die zweithäufigste Form des Heroinkonsums. Die Wirkung tritt im Vergleich zum IV-Konsum mit wenigen Minuten Verzögerung ein. Die beim IV-Konsum beobachteten übertragbaren Krankheiten wie HIV, Hepatitis oder andere Infektionen werden beim inhalativem Konsum nicht berichtet. Es kann jedoch zum Auftreten von schweren Bronchospasmen kommen. Eine zerebrale Auswirkung ist die Leukoenzephalopathie, eine spongiforme Degeneration der weissen Hirnsubstanz, die bisher nur beim inhalativen Heroinkonsum beschrieben wurde. Ursächlich soll die Pyrolysat-Bildung beim Erhitzungsprozess von Heroin beteiligt sein. Bei häufig beobachteter Polytoxikomanie und gleichzeitigem Tabakkonsum ist die Kausalität von Konsumfolgen generell schwierig zu beurteilen. Bei Auftreten einer unklaren pulmonalen Symptomatik oder Erkrankung, die sich wie ein schwerer Asthmaanfall manifestiert, sollte differenzialdiagnostisch an den inhalativen Heroinkonsum gedacht werden.
Abstract. Foil smoking is generally heroin consumption by inhalation. By heating an aluminum foil with a fire lighter the heroin on top of the foil starts melting and the smoke is then inhaled using a straw. Foil smoking is the second most common form of heroin consumption after the intravenous use. In contrast to the IV administration, heroin inhalation effects are delayed by a few minutes and the risk of transmission of HIV, Hepatitis or other infectious diseases is not relevant. Severe bronchospasms can occur in patients with or without prior pulmonary disease such as asthma bronchiale. One cerebral consequence of foil smoking is leukoencephalopathy, a spongiform degeneration of the white matter. This is likely triggered by the pyrolysate generated during the heating process. Multiple drug use and concomitant cigarette smoking in heroin addicts make cause-effect relationships difficult to assess. In general, the history of inhalative heroin consumption should be considered in patients presenting with any unkown pulmonary disease with severe bronchospasms as may happen in a severe asthma exacerbation.
Résumé. Le tabagisme sur feullie d’aluminium fait référence à la consommation d’héroïne par inhalation. Après avoir chauffé et liquéfié l’héroïne en appliquant un briquet sur la face inférieure d’une feuille d’aluminium, une paille est utilisée pour inhaler la fumée résultante. Outre la consommation connue par voie intraveineuse (IV), le tabagisme en utilisant des feuilles d’aluminium est la deuxième forme la plus courante de consommation d’héroïne. L’effet se produit avec quelques minutes de retard par rapport à la consommation IV. Les maladies infectieuses comme le VIH, l’hépatite ou d’autres infections observées lors de l’utilisation par voie intraveineuse ne sont pas signalées lors de l’utilisation par inhalation. Cependant, des bronchospasmes sévères peuvent survenir. Un effet cérébral est la leucoencéphalopathie, une dégénérescence spongiforme de la substance blanche du cerveau, cette dernière n’ayant été decrtie, qui, jusqu’à présent, que par l’inhalation d’héroïne. La formation de pyrolysats serait impliquée dans le processus de chauffage de l’héroïne. Dans le cas de la polytoxicomanie fréquemment observée et de la consommation simultanée de tabac, la causalité des conséquences de la consommation est généralement difficile à évaluer. Si une symptomatologie ou une maladie pulmonaire peu claire se manifeste par une crise d’asthme grave, l’inhalation d’héroïne devrait être envisagée dans le diagnostic différentiel.
