Polymedikation und Neuroleptika in Schweizer Pflegeheimen in den Jahren 2019 und 2020
Abstract
Zusammenfassung. Es werden Daten von 92404 klinischen Assessments aus 619 Schweizer Pflegeheimen präsentiert. Diese wurden in den Jahren 2019 und 2020 bei über 65-Jährigen erhoben. Zwei Drittel der Untersuchten wiesen gravierende Einschränkungen der Kognition auf, ein Viertel bedeutende Verhaltensauffälligkeiten sowie etwas mehr als die Hälfte Anzeichen einer emotionalen Instabilität oder Depression. 46% der Untersuchten wurden mit neun und mehr verschiedenen Arzneimitteln behandelt, 37% erhielten ein Neuroleptikum. Dieses wurde bei 85% während mehr als 90 Tagen verabreicht. Es besteht eine positive Korrelation zwischen Anzahl eingenommener Arzneimittel, Alter, Einschränkung der Kognition und Sturzanfälligkeit. Da Neuroleptika zu 30% mit unklarer Indikation verabreicht werden und die Sturzanfälligkeit unter Neuroleptika um 40% erhöht ist, sollte deren Einsatz im Rahmen strukturierter interprofessioneller Prozesse reduziert werden.
Abstract. We present data from 92404 clinical assessments from 619 Swiss nursing homes . The data were collected in 2019 and 2020 from over 65-year-old residents. Two-thirds of those studied had severe cognitive limitations, one quarter had significant behavioral disorders and just over half of the assessed patients showed signs of emotional instability or depression. 46% were treated with nine or more different drugs, 37% received a neuroleptic in 85% for more than 90 days. There is a positive correlation between the number of drugs taken, age, cognitive impairment and susceptibility to falls. Since neuroleptics are administered in 30% under uncertain indication and the susceptibility to falls is increased by 40% under neuroleptics, their use should be reduced in the course of structured interprofessional processes.
Résumé. Les données de 92404 évaluations cliniques de 619 établissements médico-sociaux suisses seront présentées. Ceux-ci ont été collectées en 2019 et 2020 auprès de pensionnaires âgés de plus de 65 ans. Les deux tiers des personnes étudiées avaient de graves limitations cognitives, un quart avait des problèmes de comportement importants et un peu plus de la moitié présentait des signes d’instabilité émotionnelle ou de dépression. 46% ont été traités avec neuf ou plus médicaments différents, 37% ont reçu un neuroleptique qui a été administré dans 85% des cas pendant plus de 90 jours. Il existe une corrélation positive entre le nombre de médicaments pris, l’âge, les limitations cognitives et la susceptibilité aux chutes. Étant donné que les neuroleptiques sont administrés dans 30% des cas avec une indication peu claire et que la susceptibilité aux chutes est augmentée de 40% sous neuroleptiques, leur utilisation devrait être réduite dans des processus interprofessionnels structurés.
