Diagnostik und Behandlungskonzept bei Hämoptoe

Unter Hämoptoe wird das Aushusten von Blut oder die Blutbeimengung im Sputum verstanden. Die Liste der Differenzialdiagnosen ist breit und hauptsächlich durch die geographische Herkunft des Patienten geprägt. Während eine leichte Hämoptoe in der Regel ambulant abgeklärt werden kann, ist die schwere Hämoptoe ein Notfall und sollte stationär in einem Zentrumsspital behandelt werden. Patienten mit schwerer Hämoptoe sollten dabei durch ein multidisziplinäres Team bestehend aus Pneumologen, Intensivmediziner/Anästhesisten, Interventionellen Radiologen und Thoraxchirurgen beurteilt und behandelt werden. Höchste Priorität kommt der Sicherung der Atemwege und der Gewährleistung des pulmonalen Gasaustausches sowie der Kreislaufstabilisierung zu. Die folgenden Abklärungs- und Behandlungsschritte bei schwerer Hämoptoe, die insbesondere die Computertomographie, Laboranalysen, die Bronchoskopie sowie allenfalls die Bronchialarterien-Embolisation umfassen, werden in Form eines Algorithmus präsentiert und diskutiert.

L'hémoptysie se définit par un crachat de sang ou strié de sang à l'occasion d'une quinte de toux. La liste des diagnostics différentiels possibles est longue et dépend de l'origine du patient. Il n'existe pas de consensus validé basé sur l'évidence concernant la définition, l'évaluation, ainsi que le traitement recommandés lors d'hémoptysie massive. Les patients concernés devraient cependant être pris en charge le plus rapidement possible par une équipe multidisciplinaire comprenant pneumologues, intensivistes/anesthésistes, radiologues interventionnels et chirurgiens thoraciques. Le maintien des voies respiratoires ouvertes et d'un échange gazeux adéquat, ainsi que la stabilisation hémodynamique, sont à effectuer avec la plus haute priorité. Les étapes diagnostiques et thérapeutiques à suivre en cas d'hémoptysie massive, à savoir le CT-scan thoracique, les analyses de laboratoire, la bronchoscopie et éventuellement l'embolisation artérielle broncho-pulmonaire, sont présentées et discutées à l'aide d'un algorithme.

Haemoptysis is defined as the expectoration of blood or bloody streaking of the sputum. The list of possible underlying differential diagnoses is broad, and the cause of haemoptysis depends on the origin of the individual patient. Patients with minor haemoptysis can often be managed in the outpatient setting. In contrast, patients with major haemoptysis require immediate in-hospital diagnostic workup and therapy. Patients with major haemoptysis should be managed by a multidisciplinary team consisting of Pulmonologists, Critical Care Physicians/Anaesthesiologists, Interventional Radiologists, and Thoracic Surgeons. In major haemoptysis, airway maintenance, adequate gas exchange, and haemodynamic stabilization are mandatory. In this article, the recommended diagnostic and therapeutic steps in patients with major haemoptysis (particularly computed tomography, laboratory analyses, bronchoscopy, and bronchial artery embolization) are presented and discussed.
