Das Urethralsyndrom: Infekt, chronischer Schmerz, Funktionsstörung und/oder psychosomatische Erkrankung?
Urethral Syndrome: Infection, Chronic Pain, Dysfunction and/or a Psychosomatic Disease?
Abstract
Zusammenfassung. Das Urethralsyndrom bei Frauen mit Schmerzen in der Harnröhre und Blasenentleerungsstörungen ist eine Ausschlussdiagnose. Zu den möglichen Ursachen zählen unter anderem Infektionen, Entzündungen der paraurethralen Drüsen, Verspannungen der Sphinkteren und/oder der Beckenbodenmuskulatur, Östrogenmangelerscheinungen, Traumata, Neuropathien, Allergien und psychosomatische Störungen. Patientinnen mit Urethralsyndrom müssen ganzheitlich betrachtet, abgeklärt und behandelt werden. Während der Abklärung und Behandlung benötigen diese Patientinnen viel Zeit und Aufmerksamkeit. Heute gilt als sicher, dass bei vielen somatischen Leiden psychosoziale Faktoren mitspielen, die den Verlauf der Erkrankung entscheidend beeinflussen können und im Behandlungsprozess berücksichtigt werden müssen.
Abstract. The urethral syndrome/urethral pain syndrome in women is a diagnosis of exclusion. It is characterized by pain in the urethra and dysfunctional micturition. Possible etiologies include organic, functional and inflammatory pathologies of the urethra. Infections, inflammation of the paraurethral glands, spasms of the sphincter muscle and or of the pelvic floor muscles, estrogen deficiency, trauma, neuropathies, hypersensitivity and/or psychosomatic issues have been investigated as possible causes. It is important to evaluate these patients as a whole. The diagnosis and treatment require time and patience. It is now well known that psychosocial aspects are involved in the development of diseases and this needs to be taken into consideration in the planning of the treatment of these patients.
Résumé. Le syndrome urétral chez la femme est un diagnostic d’exclusion. Il est caractérisé par une douleur urétrale et une dysfonction lors de la miction. Les causes possibles incluent des pathologies fonctionnelles, organiques et inflammatoires de l’urètre. Des infections, une inflammation des glandes paraurétrales, des spasmes du muscle sphinctérien et/ou des muscles du plancher pelvien, une déficience en oestrogènes, des neuropathies, une hypersensibilité et/ou des troubles psychosomatiques ont été investiguées comme causes possibles. Il est important d’évaluer chaque patiente dans sa totalité. Le diagnostic et le traitement requièrent temps et patience. Il est maintenant bien connu que des facteurs psychosociaux peuvent être impliqués dans le développement de ce syndrome et que cela doit être pris en considération dans la planification du traitement de ces malades.
