Die körperliche Untersuchung des «unkooperativen» älteren Patienten

Zusammenfassung. Die körperliche Untersuchung unkooperativer älterer Patienten stellt Ärzte in der Praxis, im Spital oder im Pflegeheim regelmässig vor grosse Herausforderungen. Der Mangel an Kooperation selbst kann dabei ein wichtiger Hinweis auf eine unterliegende medizinische Problematik sein. Wichtige Elemente, um die Kooperation des Patienten zu verbessern, beinhalten das Sicherstellen von Grundbedürfnissen, ausreichend Zeit und Geduld, eine adäquate Kommunikation sowie eine gute Zusammenarbeit mit den Angehörigen und den anderen medizinischen Fachpersonen. Eine gezielte klinische Beobachtung sowie das Denken in geriatrischen Syndromen und Bedürfnissen kann helfen, körperliche Befunde trotz eingeschränkter Kooperation zu erheben. Pathologische Befunde sind dabei Indikatoren für gestörte Organ- und Funktionssysteme und müssen unbedingt durch eine Detailuntersuchung ergänzt werden.


The Physical Examination of an ’Uncooperative’ Elderly Patient

Abstract. The physical examination of uncooperative elderly patients regularly presents physicians in the private practice, in the hospital or nursing home with great challenges. The lack of cooperation itself can be an important indication of an underlying medical problem. Important elements to improve the patient’s cooperation include ensuring basic needs, sufficient time and patience, adequate communication and good cooperation with relatives and other healthcare professionals. Targeted clinical observation as well as thinking in geriatric syndromes and unmet needs can help to raise physical findings despite limited cooperation. Pathological findings are indicators of impaired organ and functional systems and must be supplemented by a detailed examination.


Résumé. L’examen physique de patients âgés non coopératifs représente souvent un grand défi pour les médecins dans les cabinets privés, les hôpitaux et les homes. Le manque de coopération en soi peut être révélateur d’un problème médical sous-jacent. S’assurer de la satisfaction des besoins primaires, prendre suffisamment de temps et se montrer patient, communiquer de manière adéquate et s’assurer d’une bonne coopération avec les proches et les autres professionnels de la santé sont des éléments importants qui favorisent et améliorent la coopération des patients. Une observation clinique ciblée ainsi qu’une réflexion centrée sur les syndromes gériatriques et les besoins spécifiques peuvent aider à mettre en lumière des problèmes physiques malgré le manque de coopération. Les problèmes pathologiques sont des indicateurs d’une dysfonction des organes et des systèmes et doivent impérativement être complétés par un examen plus approfondi.
