Abstract
Zusammenfassung. Trotz einer überwiegend positiven Einstellung zur Organspende besitzen in der Bundesrepublik Deutschland lediglich etwa 12% der Bevölkerung einen Organspendeausweis. Somit besteht eine starke Diskrepanz zwischen der Anzahl benötigter und zur Verfügung stehender Spenderorgane. Anknüpfend an erste Arbeiten im amerikanischen Raum geht die vorliegende Arbeit den Fragen nach, welche Merkmale die Bereitschaft, Organspender zu werden, bestimmen und in welchem Maße die Konstrukte des Transtheoretischen Modells (TTM) zur Beschreibung und Erklärung des Entscheidungsprozesses zur Organspende beitragen. In einer anfallenden Stichprobe von n = 325 Personen (55% Frauen, Altersdurchschnitt 38 Jahre) wurden in einer Querschnittsbefragung soziodemographische Variablen, Einstellungen zur Organspende sowie die Operationalisierungen der Konstrukte des TTM bezogen auf das Zielverhalten “Ausfüllen eines Organspendeausweises und Mitteilen der Entscheidung in der Familie“ erhoben. Die Ergebnisse zeigen, dass 61% der Befragten noch keine Entscheidung getroffen haben und dies auch nicht in absehbarer Zeit beabsichtigen (Stufe der Absichtslosigkeit). Die Bereitschaft zur Organspende (operationalisiert durch die Stufen der Verhaltensänderung) ist wenig abhängig von soziodemographischen Merkmalen mit Ausnahme eines leichten Zusammenhanges mit religiösen Überzeugungen. Erwartungsgemäß zeigt sich, dass vor allem die Gruppe der Personen in der Stufe der Absichtslosigkeit durch vergleichsweise niedrige wahrgenommene Vorteile, eine niedrige Selbstwirksamkeit und hohe Werte für die wahrgenommenen Nachteile einer Entscheidung zur Organspende gekennzeichnet ist. Die Ergebnisse deuten an, dass der Entscheidungsprozess bzgl. der Organspende durch die Konstrukte des TTM abgebildet werden kann. Diese Ergebnisse werden vor dem Hintergrund ihrer möglichen Bedeutung für die Entwicklungen von Interventionen zur Förderung der Organspendebereitschaft diskutiert. Weitere Studien sollten insbesondere größere und bevölkerungsrepräsentativere Stichproben einbeziehen. Zudem sollten die aus dem amerikanischen übertragenen Messinstrumente noch einmal hinsichtlich der Vollständigkeit kulturspezifisch relevanter Items überprüft werden.
Abstract. While the majority of the German population is in favour of organ donation, only approx. 12% carry an organ donation card. As a consequence, there is a considerable discrepancy between the number of needed organs and the number of organs that are transplanted in Germany. This study evaluates relevant determinants for the readiness to become an organ donor. The focus lies on the significance of the constructs of the Transtheoretical Model (TTM) in this context. In a cross-sectional design, a convenience sample of n = 325 individuals (55% female, mean age: 38 years) was asked to fill out a questionnaire which included sociodemographic variables, attitudes toward organ donation and the TTM constructs (stages of change, decisional balance, self-efficacy) for the target behaviour “having an organ donation card and informing the family about the decision to become an organ donor”. Approx. 61% had not decided to become an organ donor and had no intentions to do so in the following six months (precontemplation stage). The readiness to become an organ donor was mostly unrelated to sociodemographic variables, except for religion. Individuals in the precontemplation stage had a lower self-efficacy, perceived the pros significantly less important and the cons significantly more important than individuals in the other stages of change. The results indicate that the decision process about becoming an organ donor can be described to some extent by the constructs of the TTM. The results are discussed in terms of their relevance for intervention planning. The findings should be replicated in a study with a larger and more representative sample. Additional studies should also make sure that the TTM scales, which were translations of American instruments, include all culturally relevant items for German populations.
