Gewalt in stationären Einrichtungen: gesellschaftlich sanktioniert?

Abstract
Zusammenfassung: Vielfältig sind die Definitionen und Typologien von Gewalt. Begründet wird, dass zur Untersuchung dieser Phänomene eine mehrschichtige Sichtweise auf der direkten, strukturellen und kulturellen Ebene erforderlich ist. Gewalt wird als eine vermeidbare Beeinträchtigung von grundlegenden menschlichen Bedürfnissen definiert. Institutionen sind auf Grund ihrer Struktur gefährdet, Gewalthandlungen zu ermöglichen. Angaben zur Häufigkeit des Auftretens von Gewalt in Pflegeheimen sind schwer zu erheben. Nicht zu vernachlässigen sind auch aggressive Handlungen von Heimbewohnern gegenüber dem Pflegepersonal. Medizinische Maßnahmen wie Fixierungen und Psychopharmaka können Gewalt bedeuten. Aus Untersuchungen geht hervor, dass diese zu häufig und zu sorglos eingesetzt werden. Gewalt bedeutet auch immer eine Missachtung von Menschenrechten. Zahlreich sind die Anzeichen, dass Gewalt gegen alte Menschen in Institutionen aber auch gegen das Pflegepersonal gesellschaftlich sanktioniert wird. Hingewiesen wird auf die prekäre Situation der Pflegeheime und die erheblichen Belastungen des Pflegepersonals. Weitere Beispiele sind Umgang mit dem Heimgesetz und dem Betreungsrecht sowie den mangelhaft ausgestatteten Kontrollinstanzen. Möglichkeiten von Hilfen für alte Menschen gibt es derzeit noch zu wenige.
Summary: Existing definitions and typologies of violence and maltreatment are quite diverse. We propose that a multidimensional approach, including consideration of direct, structural and cultural dimensions, is needed for an understanding of these phenomena. Maltreatment is defined as an infringement of basic human needs. Because of their inherent structure, institutions are prone to downplay occurrences of maltreatment. The frequency of such events in nursing homes is therefore difficult to determine. An evaluation of this problem requires consideration of aggressive behavior of nursing home residents against staff as well. Medical interventions such as mechanical restraints or psychoactive drugs may be a type of maltreatment. The literature provides evidence for an excessive and inappropriate use of both. Maltreatment always involves infringement of basic human rights. There is ample evidence that maltreatment of nursing home residents, and also of nursing home staff, is sanctioned socially. We point to the precarious situation in nursing homes and the stressful working conditions for the staff. Other examples include the inadequate handling of supervision and control mandated under the Nursing Home Law and the Guardianship Law. To minimize elder maltreatment, an active collaboration of all involved in their care is needed.
