Abstract
Zusammenfassung: Zur Diagnose eines Neglects werden im klinischen Alltag häufig Durchstreichaufgaben oder das Linienhalbieren angewendet. Die vorliegende Arbeit gibt einen Überblick darüber, wie treffsicher diese Verfahren einen Patienten mit Neglect identifizieren können. Es zeigt sich, daß die Durchstreichaufgaben, insbesondere der “Letter cancellation Test” und der “Bells Test”, die höchsten Trefferquoten aufweisen. Mit der Linienhalbierungsaufgabe werden hingegen ca. ein Drittel der Patienten mit Neglect fälschlicherweise nicht diagnostiziert. Darüber hinaus sind Defizite im Linienhalbieren nicht spezifisch für Patienten mit Neglect. Auch andere Patientengruppen, wie etwa Patienten mit Hemianopsie, zeigen Abweichungen bei dieser Aufgabe. Demgegenüber weisen bei Durchstreichaufgaben allein Patienten mit einem Neglect lateralisierte Defizite auf. Wir schlußfolgern, daß beide Aufgabentypen nicht gleichermaßen für die Diagnose einer Neglect-Symptomatik geeignet sind. Im Gegensatz zu den Vernachlässigungen bei Durchstreichaufgaben erlaubt. Das Abweichen bzw. Nicht-Abweichen beim Linienhalbieren erlaubt keinen sicheren Schluß auf das Vorliegen bzw. Nicht-Vorliegen eines Neglects.
Summary: In a clinical context, spatial neglect is usually assessed with tests like cancellation or line bisection. Reflecting previous studies and own results, this review article compares line bisection and cancellation tasks regarding their accuracy in detecting spatial neglect. It turned out that the line bisection test missed almost one third of patients from a well defined population with spatial neglect. Far superior were the “letter cancellation” and the “bells tests”. Moreover, deficits in line bisection are not specific for neglect patients. Performance in line bisection is influenced by factors other than neglect, e.g. hemianopia, whereas lateralized omissions in cancellation tests are only found in patients suffering from spatial neglect. We conclude that line bisection and cancellation are not similarly suitable for the clinical diagnosis of neglect. Performance in line bisection is not a reliable predictor of spatial neglect.
