Unsere Memory Clinic am Tagesspital hat sich aus einem seit Jahren bestehenden Angebot für ambulante Gedächtnisabklärung entwickelt. In diesem Artikel wird dargestellt, wie eine solche zwei- bis dreitägige ambulante Abklärung konkret vor sich geht: Aufnahmegespräch mit Einführung im Tagesspital, Erfassen der Alltagsaktivitäten, semistandardisiertes Interview von Patienten und Betreuern, neuropsychologische Tests, somatische Untersuchung mit funktionellem Assessment. Im Folgenden wird dargestellt, wie aus diesen Befunden die Diagnose erarbeitet wird. Häufige Fragestellungen und diagnostische Schwierigkeiten werden dabei eingehender beleuchtet. Weitere Schwerpunkte sind die Diagnose- und Therapiebesprechung sowie der Inhalt des Beratungsgespräches.

Our Memory Clinic at the Day Hospital has developed from years of existing services offered for the outpatient diagnosis of memory disorders. This article presents how such two- to three-day outpatient diagnostic programs are held. After an initial consultation, the patient is admitted to the day clinic. Then a survey of the patient's daily activities is taken, a semi-standardized interview is conducted with patients and the caregivers; neuropsychological tests and a somatic examination including a functional assessment are carried out. In the following, we will discuss how the diagnosis is subsequently rendered on the basis of these findings. Frequently asked questions, common problems and diagnostic difficulties will be elucidated, while focusing on diagnostic and therapeutic discussions and the contents of the consultations.

Notre clinique de jour de la mémoire s'est développée sur la base d'une offre existant déjà depuis des années pour investiguer en ambulatoire la mémoire. Ce travail décrit de façon concrète le déroulement d'une investigation ambulatoire de deux à trois jours: entretien de prise en charge avec présentation de la clinique de jour, évaluation des activités quotidiennes, interview semi-standardisé des patients et des soignants, tests neuro-psychologiques, examen somatique avec évaluation fonctionnelle. Par la suite, il est démontré comment à partir de ces résultats un diagnostic peut être posé. Les questions fréquentes et les difficultés diagnostiques sont abordées de façon plus approfondie. D'autres points importants sont la discussion du diagnostic et du traitement ainsi que le contenu de la discussion de conseils médicaux.
