Charakteristika der Alkohol- erkrankung bei Bundeswehrsoldaten in der stationären Versorgung im Vergleich zu zivilen Patienten
Abstract
Fragestellung: Stationäre Behandlungssettings in der Suchtmedizin sollten Zielgruppen-orientiert ausgerichtet sein. Diese Studie untersucht, ob spezifische Charakteristika alkoholkranker Bundeswehrsoldaten in der stationären Akutversorgung im Vergleich zu zivilen Patienten abgrenzbar sind. Methodik: 158 Patienten, die im Bundeswehrkrankenhaus Berlin aufgrund einer Alkoholerkrankung stationär behandelt wurden (115 Soldaten, 43 Zivilpatienten) wurden retrospektiv bzgl. der Erkrankungsdauer, Diagnose und Trinkmenge evaluiert und mit einer stationär-psychiatrischen militärischen Kontrollgruppe (n = 46) im Hinblick auf psychosoziale, medizinische und militärische Merkmale verglichen. Ergebnisse: Zivile Patienten wiesen (alterskorreliert) eine im Mittel signifikant längere Trinkdauer, höhere Trinkmenge und häufiger eine Alkoholabhängigkeit auf als militärische Patienten. Familiäre Konflikte standen signifikant mit einer höheren, Konflikte im militärischen Umfeld mit einer geringeren Trinkmenge in Verbindung. Schlussfolgerungen: Die Charakteristika von Bundeswehrsoldaten in der stationären Akutversorgung der Alkoholerkrankung unterscheiden sich von zivilen Patienten und bilden damit den in internen Richtlinien der Bundeswehr festgelegten frühen Behandlungsbeginn bereits bei schädlichem Gebrauch ab. Dies sollte in der Gestaltung militärbezogener therapeutischer Settings verstärkt berücksichtigt werden.
Aims: Inpatient treatment of addictive disorders should be adapted to the needs of target groups. This study investigates specific characteristics of acute inpatient Bundeswehr alcohol use disorder patients. Methods: 158 patients (115 soldiers, 43 civil) were inpatients in the Bundeswehr hospital Berlin for alcohol disorders (ICD-10: F10.1/F10.2). They were evaluated retrospectively for duration of abuse, diagnosis and average sonsumption and compared with an inpatient military control group with respect to psychosocial, military and medical characteristics. Results: Civil patients had (as an age-related effect) a significantly longer duration of alcohol abuse, higher average consumption and more often the diagnosis of an alcohol dependence than their military counterparts. Family conflicts correlated significantly with higher, and problems with military comrades or superiors with lower alcohol intake. Conclusions: The results suggest differences in the history of alcohol consumption between military and civil patients in an acute inpatient treatment setting and thus reflect the early onset of alcohol-related interventions in the German Armed Forces due to military regulations. An adaption of therapeutic programs for soldiers might be useful.
