Mögliche Aggravation einer Purpura Schönlein-Henoch durch Isotretinoin

Wir berichten über einen 16-jährigen Patienten mit einer hämorrhagisch-bullösen Form einer Purpura Schönlein-Henoch, deren Ursache nicht eruiert werden konnte, und die wie erwartet komplikationslos abheilte. Da der Patient wegen einer Akne inversa drei Wochen zuvor eine Therapie mit Isotretinoin begonnen hatte, konnte ein möglicher Zusammenhang mit dieser Therapie nicht ausgeschlossen werden. Der Fall wurde an die Swissmedic gemeldet. Im Kommentar wird auf Isotretinoin, die medikamentös induzierte Vaskulitis und ihre Pathomechanismen eingegangen. Als allgemeiner Reminder werden die Kriterien zur Beurteilung der Kausalität einer unerwünschten Arzneimittelwirkung (UAW) sowie das Spontanmeldewesen für UAW in der Schweiz erörtert.

Nous rapportons l'histoire d'un patient de 16 ans souffrant d'une forme hémorragique bulleuse de la maladie de Henoch-Schönlein, dont la cause n'a pas pu être trouvée et qui a guéri sans complications comme prévu. Puisque le patient avait commencé une thérapie par l'isotretinoïne à cause d'une acné inversa au cours des 3 semaines précédentes, un lien possible ne pouvait pas être exclu. Le cas a été annoncé à Swissmedic. Dans le commentaire nous parlons de l'isotretinoïne, de la vascularite d'origine médicamenteuse et leurs mécanismes physiopathologiques. Comme rappel général, les critères pour le jugement de la causalité des effets indésirables médicamenteux ainsi que le système d'annonce des effets indésirables médicamenteux en Suisse sont discutés.

We report on a 16 year old patient with a hemorrhagic bullous form of a Henoch-Schönlein purpura. The cause for the event could not be determined, but the patient recovered without complications. Since he had begun a therapy with isotretinoin due to an acne inversa 3 weeks before, a possible association between the event and the treatment could not be excluded, and the case was reported to Swissmedic. In the comment, we discuss the pharmacology of isotretinoin, drug-induced vasculitis, and its pathomechanisms. As a general reminder, the criteria for the causality assessment of adverse drug reactions as well as the reporting system of adverse drug reactions in Switzerland are discussed.
