Antipsychotika im Kindes- und Jugendalter: pro und contra
Abstract
Zusammenfassung. Das Indikationsspektrum für Antipsychotika hat sich in den letzten Jahren vergrössert und umfasst inzwischen nicht nur die Behandlung von Psychosen, sondern auch die Behandlung von impulsiv-aggressivem Verhalten bei intelligenzgeminderten Kindern und Jugendlichen mit Störungen des Sozialverhaltens oder Autismus-Spektrum-Störungen. Dadurch ist es weltweit zu einer Zunahme von Verschreibungen gekommen, die sehr kritisch diskutiert wird. In diesem Beitrag werden das Indikationsspektrum und die potenziellen Nebenwirkungen von Antipsychotika bei der Behandlung von Kindern und Jugendlichen dargestellt und die mit der Medikation verbundenen Risiken dem möglichen Nutzen gegenübergestellt, um zu einer rationalen Behandlungsempfehlung zu gelangen.
Abstract. In the last decade the indication of antipsychotics has been expanded from the treatment of psychoses to the treatment of impulsive-aggressive behaviors in mentally retarded children and adolescents with conduct disorder or autism spectrum disorders. As a consequence the use of antipsychotics in children and adolescents has increased worldwide. This increase of prescriptions is under critical discussion. In this paper the indication and the potential side-effects of antipsychotics in children and adolescents are described. The risks of antipsychotic medication are contrasted with the potential benefits to arrive at rational treatment recommendations.
Résumé. Au cours de la dernière décade l’indication des antipsychotiques s’est élargie du traitement des psychoses au traitement des comportements impulsifs-agressifs chez les enfants retardés sur le plan mental et les adolescents présentant des troubles du comportement ou du spectre de l’autisme. Comme conséquence l’emploi des antipsychotiques chez les enfants et les adolescents s’est accru dans le monde entier. Cette augmentation des precriptions est discutée de manière critique. Dans ce papier sont décrits l’indication et les effets secondaires des antipsychotiques chez l’enfant et l’adolescent. Les risques de ces médicaments sont mis en regard avec leurs bénéfices potentiels afin de parvenir à des recommandations de traitement rationnelles.
