Abstract
Zusammenfassung. Unterschenkelfrakturen gehören zu den häufigsten Schaftfrakturen bei Kindern und werden mehrheitlich konservativ behandelt. Die Analyse der Frakturmorphologie und Kenntnis des Remodeling-Potenzials des kindlichen Skeletts sind wichtige Voraussetzungen zur Einleitung der korrekten Therapie. Die konservative Therapie besteht aus einer Gipstherapie über drei bis sechs Wochen je nach Alter des Kindes. Zudem besteht die Möglichkeit, über eine Gipskeilung das Repositionsresultat günstig zu beeinflussen. Die Indikation für eine operative Therapie beschränkt sich auf stark dislozierte oder instabile sowie offene Frakturen. Im Rahmen der Nachkontrollen werden sekundäre Dislokationen sowie Wachstumsstörungen erkannt und behandelt.
Abstract. Fractures of the lower leg are among the most frequent shaft fractures in children. The majority of these fractures is treated non-operatively. A thorough analysis of fracture patterns and knowledge of the remodeling potential is mandatory for a successful treatment. The nonoperative treatment consists of a cast application over three to six weeks, according to the child’s age. With cast wedging, the anatomic fracture alignment can be optimized. Surgical treatment is established only for severe fracture displacement and for unstable and open fractures. Follow-up is mandatory to detect secondary fracture displacement and growth disturbances.
Résumé. Les fractures diaphysaires de la jambe sont parmi les plus fréquentes chez les enfants. Dans la plupart des cas elles peuvent être traitée conservateur. L’analyse de la morphologie de la fracture et la connaissance du potentiel du remodelage du squelette chez l’enfant sont des conditions préalables pour initier une thérapie correcte. Le traitement conservateur consiste d’un plâtre pour six semaines. En plus, il existe la possibilité d’une gypsotomie pour améliorer le résultat de la réduction. Le traitement operateur se limite aux fractures fortement déplacé es, instables ou ouvertes. Au suivi clinique, il est important de détecter des déplacements secondaires ainsi que des retards de la croissance.
