Abstract
Zusammenfassung. Wir berichten über einen 83-jährigen Patienten mit kardialer Synkope, Schädel-Hirn-Trauma und intrakranieller Blutung unter Antikoagulation. Bei dem Patienten entwickelte sich eine «medizinische Zwickmühle» aufgrund des Sturzes und bestehender chronischer Vorerkrankungen. Vorgängig nicht diagnostizierte kardiale Grunderkrankungen führten in diesem Fall zu einer Synkope. Die konsekutiven intrakraniellen Blutungen wurden durch die aufgrund rezidivierender unprovozierter Lungenembolien indizierte Antikoagulation begünstigt. Die Blutungen selbst verhinderten eine kausale Behandlung der kardialen Grundproblematik, was im Falle dieses Patienten nach mehreren Komplikationen zum Tode führte.
Abstract. Here we report on a 83 year-old patient with cardiac syncope and consecutive traumatic brain injury with intracranial haemorrhage receiving anticoagulation for recurrent pulmonary embolism: a ’medical dilemma’ due to the syncope with consecutive traumatic event and the underlying condition. A pre-existing underlying cardiac disease was identified as the cause of the syncope and the intracranial haemorrhage was most likely due to oral anticoagulation for recurrent pulmonary embolisms. The intracranial bleeding inhibited an optimal management of the underlying cardiac condition and the patient deceased shortly thereafter.
Résumé. Nous rapportons le cas d’un patient de 83 ans ayant présenté une syncope d’origine cardiaque, un traumatisme crâniocérébral et une hémorragie intracrânienne sous anticoagulation. Un dilemme médical est apparu chez ce malade étant donné la chute et les affections chroniques préexistantes. Dans ce cas une atteinte cardiaque non décelée auparavant a provoqué une syncope. L’hémorragie intracrânienne qui a suivi a été favorisée par l’anticoagulation en cours motivée par des embolies pulmonaires à répétition. L’hémorragie a rendu impossible un traitement causal du problème cardiaque de base qui a conduit au décès du patient après quelques complications.
