Depression und Gewalt: Ein Widerspruch?
Abstract
Zusammenfassung. Depressionen gehören weltweit zu den führenden Erkrankungen. Sie sind von hoher Bedeutung für unterschiedlichste Bereiche innerhalb der Medizin, da Betroffene häufig ihren Hausarzt oder Internisten zuerst aufsuchen. Obwohl Symptome einer Depression sowohl bei Frauen als auch bei Männern beobachtet werden können, gibt es Besonderheiten der Symptomkonstellation bei Männern, die bislang nur unzureichend bekannt sind bzw. beachtet wurden. Diese Arbeit widmet sich dem Konzept der «Male Depression», das unter anderem durch sozialen Rückzug, Reizbarkeit oder Substanzmissbrauch gekennzeichnet ist. In diesem Kontext bestehen praxisrelevante Zusammenhänge zwischen Depression und gewalttätigem Verhalten, die anhand von zwei Fallbeispielen veranschaulicht werden sollen. Von besonderer Brisanz ist dabei das sogenannte suizidal-homizidale Syndrom, das zwingend einer weiteren psychiatrischen, wenn nicht sogar einer forensisch-psychiatrischen Abklärung zugeführt werden sollte.
Abstract. Depression is one of the leading diseases worldwide. Its symptoms are of great importance for a variety of areas within internal medicine, as patients often pay a visit to their general practitioners first. Although symptoms of depression are observed in both genders, existing peculiarities of the symptoms in men are only insufficiently known or acknowledged. Therefore, we dedicate this article to the concept of male depression, which can be characterized by social withdrawal, irritability or substance abuse. Here, we present practice-related associations between depression and violent behavior, which will be illustrated by two case studies. Of particular relevance is the so-called suicidal-homicidal syndrome, which should make further psychiatric, if not forensic-psychiatric evaluation mandatory.
Résumé. La dépression est l’une des principales maladies dans le monde. Les symptômes de dépression revêtent une grande importance pour la médecine, parceque les patients consultent principalement leur médecin de famille. Bien que les symptômes puissent être observés dans les deux sexes, il y a des particularités des symptômes chez les hommes qui jusqu’à présent n’étaient qu’ insuffisamment connus. Ce travail est dédié au concept de dépression masculine, caractérisée par le retrait social, l’irritabilité ou la toxicomanie. Dans ce contexte, on observe en pratique clinique une relation entre la dépression et le comportement violent ce qui sera illustré par deux études de cas. Le syndrome suicidaire-homicide, qui devrait être obligatoire pour toute évaluation psychiatrique ultérieure, voire médico-légale, est particulièrement pertinent.
