Delirmanagement in der Palliative Care
Abstract
Zusammenfassung. Delir ist eine der häufigsten neuropsychiatrischen Komplikationen bei Personen mit einer fortgeschrittenen unheilbaren Erkrankung. Delir am Lebensende tritt häufig auf, wird aber oft übersehen, nicht diagnostiziert oder falsch/nicht behandelt. Delirien sind auch bei palliativer Situation – so weit möglich – zu behandeln, weil persistierende delirante Zustände die Fragilität der Patientinnen und Patienten erhöhen, die körperliche Funktionalität einschränken und die Lebenszeit verkürzen. Zudem lösen akute Verwirrtheitszustände hohen Distress bei Betroffenen und deren Angehörigen aus, beeinträchtigen die Lebensqualität und verhindern ein würdevolles Sterben. Während Halluzinationen und Visionen am Lebensende in der Medizin als Delirium interpretiert und als solches behandelt werden, wird dieses Phänomen von der philosophischen und theologischen Hermeneutik als eine Ressource interpretiert, die den Betroffenen und deren Angehörigen helfen kann, sich mit vergangenen Lebensereignissen zu versöhnen und sich mit dem Sterbeprozess auseinanderzusetzen. Das Auftreten von Visionen am Lebensende in Abgrenzung zum Delir ist jedoch noch wenig untersucht und bedarf einer genaueren Exploration.
Abstract. Delirium is one of the most common neuropsychiatric complications in patients with advanced incurable disease. End-of-life delirium is common but is often overlooked, undiagnosed or incorrectly diagnosed/untreated. Delirium should also be treated in a palliative situation – as far as possible – because persistent delirious states increase the patient’s fragility, limit physical functionality and shorten the lifespan. In addition, acute states of confusion trigger high levels of distress in affected patients and their relatives, impair the quality of life and a dignified dying process. While hallucinations and visions at the end of life are interpreted as delirium in medicine and treated as such, this phenomenon is interpreted by philosophical and theological hermeneutics as a resource that can help patients and their relatives to reconcile with past life events and to deal with the process of dying. However, the occurrence of end-of-life visions as opposed to delirium has not yet been studied very much and requires more detailed exploration.
Résumé. Le délire est l’une des complications neuropsychiatriques les plus fréquentes chez les patients atteints d’une maladie incurable à un stade avancé. Le délire de fin de vie est fréquent, mais il est souvent négligé, non diagnostiqué ou incorrectement diagnostiqué/non traité. Le délire doit également être traité en situation palliative – dans la mesure du possible – car les états délirants persistants augmentent la fragilité du patient, limitent la fonctionnalité physique et raccourcissent la durée de vie. De plus, les états aigus de confusion déclenchent des niveaux élevés de détresse chez les patients touchés et leurs proches, nuisent à la qualité de vie et empêchent de mourir dans la dignité. Alors que les hallucinations et visions de fin de vie sont interprétées comme du délire en médecine et traitées comme telles, ce phénomène est interprété par l’herméneutique philosophique et théologique comme une ressource pouvant aider les patients et leurs proches à se réconcilier avec les événements de la vie passée pour faire face au processus de mourir. Cependant, la survenue de visions de fin de vie par opposition au délire n’a pas encore été bien étudiée et nécessite une exploration plus détaillée.
