Abstract
Zusammenfassung. Palliative Care wird normalerweise nicht mit dem Kindes- und Jugendalter in Verbindung gebracht. Trotzdem leben allein im deutschsprachigen Europa mehr als 50 000 Kinder mit lebensverkürzenden Krankheiten und jährlich sterben etwa 5000 Kinder. Palliative Care für Kinder und Jugendliche (PPC) ist eine Haltung, die niemandem aufgezwungen werden darf, sondern gelebt werden muss; bei komplex-chronischen Krankheiten oftmals über viele Jahre. Eine erfolgreiche und nachhaltige Implementierung von PPC basiert deshalb auf einer wertschätzenden Zusammenarbeit zwischen verschiedenen Teams und den betroffenen Familien getreu der hier vorgestellten Quadriga einer umfassenden Symptomkontrolle mit Neugier, Geduld, Aktivität und persönliche Zurückhaltung. Notwendige Grundlagen wie der Betreuungsplan, der interprofessionelle Hausbesuch und der runde Tisch sowie die 24-Stunden-Erreichbarkeit werden ebenso vorgestellt wie eine Triage-Technik und Advanced Care Management.
Abstract. Palliative care is not usually associated with childhood and adolescence. Nevertheless, in German-speaking Europe alone, more than 50 000 children live with life-shortening illnesses, and about 5000 children die every year. Palliative care for children and adolescents (PPC) is an attitude that must not be forced on anyone, but must be lived; and, in the case of complex-chronic diseases often over many years. A successful and sustainable implementation of PPC is therefore based on an appreciative collaboration between different teams and the affected families, true to the quadriga presented here of comprehensive symptom control with curiosity, patience, activity and humility. Necessary basics such as the care plan, interprofessional home visits and the round table, plus 24-hour accessibility, are here presented, as well as a triage technique and the advanced care management.
Résumé. Les soins palliatifs ne sont normalement pas associés à l’enfance et à l’adolescence. Néanmoins, rien que dans l’Europe germanophone, plus de 50 000 enfants vivent avec des maladies qui raccourcissent leur vie et environ 5000 enfants meurent chaque année. Les soins palliatifs pour les enfants et les adolescents (PPC) sont une attitude qui ne doit être imposée à personne, mais qui doit être vécue; dans le cas de maladies chroniques complexes, souvent pendant de nombreuses années. La mise en œuvre réussie et durable du PPC repose donc sur une coopération appréciative entre les différentes équipes et les familles concernées, fidèle au quadrige présenté ici de contrôle global des symptômes avec curiosité, patience, activité et humilité. Les bases nécessaires telles que le plan de soins, les visites interprofessionnelles à domicile et la table ronde avinsi que l’accessibilité 24h/24 sont présentées ainsi qu’une technique de triage et la gestion des soins avancés.
