Abstract
Zusammenfassung. Die vorliegende Studie untersucht Transfereffekte eines schulischen Gesundheitsprogramms zur Prävention von Alkohol- und Medikamentenkonsum auf den Bereich der Rauchprävention. Zur Abschätzung der Programmwirkungen wurden persönliche und soziale Schutz- bzw. Risikofaktoren (Rauchverhalten der Freunde, Eltern und Geschwister; rauchbezogene Instrumentalitäts- und Selbstwirksamkeitserwartungen; Selbstwertgefühl) als zusätzliche Prädiktoren berücksichtigt. Die Evaluation folgt einem quasi-experimentellen Versuchsplan mit 22 Gesamtschulklassen der siebten bis neunten Klassenstufen in einer unstandardisierten und einer standardisierten Interventionsgruppe sowie einer Kontrollgruppe. Für 357 Schüler (189 Mädchen und 168 Jungen; Alter: 13 bis 18 Jahre) liegen vollständige Daten zu Prätest, Posttest und Follow-up vor. Die mittels logistischer Regressionen ermittelten Ergebnisse zeigen, daß aus primärpräventiver Sicht das Rauchverhalten der engsten Freunde und die rauchbezogenen Wirksamkeits- bzw. Instrumentalitätserwartungen entscheidend dafür sind, ob nicht rauchende Jugendliche mit dem Rauchen beginnen oder nicht, während die Intervention keine zusätzlichen Effekte erzielte. Eine sekundärpräventive Wirkung des Gesundheitsprogramms ergab sich in der standardisierten Interventionsgruppe, in der bedeutsam mehr Jugendliche mit dem Rauchen aufhörten als in der unstandardisierten Interventionsgruppe und der Kontrollgruppe. Einflüsse der persönlichen und sozialen Faktoren gab es hier hingegen nicht. Die Befunde werden diskutiert im Hinblick auf unterschiedliche Risikogruppen, Präventionsziele und psychosoziale Determinanten für den Beginn und die Einstellung des Rauchverhaltens.
Abstract. A school-related health program designed to prevent alcohol and drug abuse is evaluated regarding possible transfer effects on smoking prevention. Personal and social risk factors (smoking behavior of friends, parents and siblings; self-esteem; resistance self-efficacy and outcome expectancies with respect to smoking) are considered as further predictors. The evaluation study is based on a quasi-experimental design with 22 comprehensive school classes (grades seven to nine) in an unstandardized and a standardized intervention group as well as a control group. Complete data at pretest, posttest and follow up are available for 357 pupils (189 girls and 168 boys; age: 13 to 18 years). Regarding primary prevention, the smoking behavior of close friends and smoking-related cognitions turned out to be dominant predictors of whether non-smoking adolescents start to smoke or not, whereas the intervention did not have any effects. On the other hand, with respect to secondary prevention, the health program was effective in so far as in the standardized intervention significantly more adolescents stopped smoking than in the unstandardized intervention group and the control group. In this case, however, no influences of social and/or personal risk factors on smoking behavior were found. The results are discussed with respect to different risk groups, prevention aims and psychosocial determinants of the onset and offset of smoking behavior.
