Instrumentelle Bewegungsaktivitäten von Herzinfarktpatienten
Ein integratives Modell zur Motivation und Volition
Abstract
Zusammenfassung. Die Initiierung regelmäßiger Sportaktivitäten von Herzinfarktpatienten nach ihrer Anschlussheilbehandlung (AHB) bildet eine wesentliche Voraussetzung zur Sicherung eines langfristigen Rehabilitationserfolges. Im Sinne einer theoretischen Rekonstruktion solcher instrumenteller Bewegungsaktivitäten wird ein integratives Modell vorgestellt. Darin werden mit der Rubikontheorie der Handlungsphasen (Heckhausen, 1989; Gollwitzer, 1996) eine handlungspsychologische Perspektive und mit sozial-kognitiven Determinanten aus Modellen zum Gesundheitsverhalten (z.B. Fuchs, 1997) eine gesundheitsspezifische Perspektive miteinander verknüpft. Zur empirischen Anwendung des Modells werden in einer Feldstudie 135 Herzinfarktpatienten während ihrer stationären AHB schriftlich und drei Monate poststationär telefonisch hinsichtlich der Initiierung regelmäßiger Sportaktivitäten im Alltag befragt. Ein zentrales Ergebnis der Studie deutet auf die besondere Rolle volitionaler Merkmale (z.B. Realisierungsintentionen, Selbstwirksamkeitserwartungen) bei der Initiierung regelmäßiger Sportaktivitäten hin. Abschließend werden neben untersuchungsmethodischen Aspekten die Ergebnisse im Hinblick auf die theoretische Weiterentwicklung des integrativen Modells sowie die Gestaltung von Interventionen zur Förderung regelmäßiger Sportaktivitäten diskutiert.
Abstract. The initiation of regular health-related physical activities is an essential condition to achieve beneficial long-term effects after phase-II cardiac rehabilitation programs. In this article, we present a theoretical model of instrumental physical activities. The model integrates ideas of the Rubicon theory (cf. Gollwitzer, 1996) with social-cognitive determinants of well-known models of health behavior (e.g., Fuchs, 1997). To test this integrative model empirically, we conducted a field study with 135 myocardial infarction patients. During their phase-II rehabilitation program the patients were asked about their perceptions of motivational and volitional determinants of physical activity. Three months later they were interviewed via telephone about their current physical activities. The main result of this study is that the volitional determinants (e.g., implementation intentions, self-efficacy) are of particular relevance to the initiation of health-related physical activities after phase-II cardiac rehabilitation programs. The results are discussed with respect to further theoretical implications and the development of intervention strategies to promote physical activity.
