Abstract
Zusammenfassung. Wir berichten über eine 79-jährige Patientin mit rezidivierenden (prä)synkopalen Zuständen über einen Zeitraum von 15 Jahren, begleitet von einem vermehrten Harndrang und häufigem nächtlichen Wasserlösen, welche bei unklarem Beschwerdebild auf die Psychosomatik zugewiesen wurde. Aufgrund fehlender neurologischer Defizite und vielfältigen weiterführenden, jedoch nicht zielführenden Abklärungen konnten wir bei typischer Anamnese, pathologischem Orthostasetest und charakteristischen Katecholaminbefunden im Plasma die Diagnose einer reinen Dysautonomie stellen. Eine psychosoziale Belastungssituation und eine psychiatrische Erkrankung konnten als relevante Ursachen für die Symptomatik ausgeschlossen werden.
Abstract. We report a 79-year-old female patient with recurrent episodes of presyncope/syncope over 15 years, accompanied by urge incontinence and nocturnal enuresis, referred to the psychosomatic ward, as her medical symptoms were unexplained. Based on a lack of neurological deficits, extensive clinical examinations without any diagnostic cues, a typical history, orthostatic challenge and plasma catecholamine findings, we were able to diagnose pure autonomic failure. We could exclude psychosocial stress or a mental disorder as relevant causes for the symptoms.
