Abstract
Abstract. Background: Ultrasound is increasingly used in clinical practice as a bedside tool. As medical graduates first encounter the technique in early residency, ultrasound training needs to be integrated into the undergraduate curriculum. In Switzerland, abdominal ultrasound skills have been taught by a faculty-led, 21-hour course. However, this course does not have sufficient capacity to meet the increasing demand, and there have been doubts about its effectiveness as a teaching method. We therefore developed a 21-hour blended-learning course, comprising five hours of e-learning and 16 hours of near-peer tutoring. This study investigates whether this new teaching format is as good as, or superior to, the faculty-led method. Methods: The SIGNATURE study is an investigator-initiated, two-arm, randomised controlled trial, enrolling 152 medical students at the Universities of Bern, Fribourg and Zurich. Stratified by study site, students are 1:1 randomised to either the blended-learning course or the faculty-led 2.5-day ultrasound course. Students undergo a six-station objective structured clinical examination (OSCE) and complete an online questionnaire immediately after the course and 6 months later. Discussion: If demonstrated to be effective, the blended-learning course would allow an increase in the number of undergraduate medical students that can acquire ultrasound skills before starting their residencies.
Zusammenfassung. Ausgangslage: Die Sonografie ist zu einem wichtigen Instrument in der klinischen Tätigkeit geworden, das bereits von Assistenzärztinnen und -ärzten in der frühen Weiterbildung eingesetzt wird. Die Ultraschallausbildung sollte daher in die Ausbildung integriert werden. In der Schweiz wurde die Basisultraschallausbildung bisher in einem 21 Stunden dauernden Kurs vermittelt. Dieser Kurs genügt der zunehmenden Nachfrage nicht, und es bestehen Zweifel bezüglich der Effektivität. Aus diesem Grund entwickelten wir einen 21 Stunden dauernden Blended-Learning-Kurs, der fünf Stunden e-Learning und 16 Stunden Near-Peer-Tutoring beinhaltet. Diese Studie untersucht, ob das neue Ausbildungsformat gleich gut oder besser ist als der herkömmliche Kurs. Methodik: Die SIGNATURE-Studie ist eine von den Untersuchern initiierte, zwei-armige, randomisiert kontrollierte Studie, die 152 Medizinstudierende der Universitäten aus Bern, Fribourg und Zürich einschliesst. Die Teilnehmer werden pro Standort 1:1 in die Blended-Learning oder in den herkömmlichen Kurs randomisiert. Die Studenten/Innen absolvieren eine sechs Stationen umfassende OSCE (Objective Structured Clinical Examination) und beantworten einen Fragebogen unmittelbar nach dem Kurs und sechs Monate danach. Diskussion: Wenn das Blended-Learning-Format sich als gleich gut oder besser als die herkömmliche Ausbildung herausstellt, könnte das Angebot für Medizinstudierende rasch ausgebaut werden. Dies würde Schweizer Medizinstudierenden erlauben, sich Ultraschallfähigkeiten vor ihrer Tätigkeit als Assistenzärztinnen und -ärzte anzueignen.
Résumé. Contexte: L’échographie est un outil de plus en plus utilisé en clinique et avec lequel les médecins-assistant(e)s n’entrent en contact qu’au début de leur formation continue. Ainsi, la formation ultrasonographique devrait être intégrée dans la formation de base. En Suisse, la formation de base en échographie a été enseignée jusqu’à présent dans le cadre d’un cours d’une durée de 21 heures. Cependant, ce format de cours ne répond pas à la demande croissante et des doutes persistent quant à l’efficacité de la méthode d’enseignement. De ce fait, nous avons développé un cours d’apprentissage mixte de 21 heures intitulé «blended-learning», qui contient cinq heures de formation en ligne (e-learning) et 16 heures de tutorat par des pairs (near-peer-tutoring). Cette étude analyse si le nouveau format de formation est équivalent ou meilleur par rapport au cours traditionnel. Méthode: L’étude SIGNATURE est un essai contrôlé-randomisé à deux bras, initié par les examinateurs et qui implique 152 étudiant(e)s en médecine des Universités de Berne, Fribourg et Zurich. Stratifié(e)s par site, les étudiant(e)s sont randomisé(e)s 1:1 en un groupe «blended-learning» et un groupe «cours traditionnel». Les étudiant(e)s devront participer à un ECOS (Examen Clinique Objectif Structuré) de six postes et remplir un questionnaire en ligne immédiatement après le cours ainsi que six mois plus tard. Discussion: Si le cours «blended-learning» se révèlait aussi bon ou meilleur que le cours existant, l’offre pour les étudiant(e)s en médecine pourrait être rapidement étendue. Cela permettrait aux étudiant(e)s suisses d’acquérir des compétences en échographie avant leur activité clinique en tant que médecin-assistant(e).
