Abstract
Zusammenfassung. Viele Befunde sprechen dafür, dass Kinder und Jugendliche mit einer funktionellen Harninkontinenz durch ihre Erkrankung stark belastet sein können. Da die Stressverarbeitungskompetenzen eine moderierende Rolle bei den Effekten der Alltagsstressoren auf den Krankheitsverlauf haben, sollte in der Studie untersucht werden, ob sich die Stressverarbeitung in Abhängigkeit von der Gruppe (funktionelle Harninkontinenz vs. gesunde Normierungsstichprobe) und vom Geschlecht unterscheidet. Außerdem sollte geprüft werden, ob die Lebensqualität der einnässenden Kinder mit der Verarbeitung alltäglicher und krankheitsspezifischer Stressoren zusammenhängt. Die klinische Stichprobe bestand aus 78 Kindern mit funktioneller Harninkontinenz im Alter von 8 bis 13 Jahren. Die Kinder bearbeiteten eine modifizierte Version des Stressverarbeitungsfragebogens für Kinder und Jugendliche (SVF-KJ) von Hampel, Petermann und Dickow (2001) und den Kid-KINDLR von Ravens-Sieberer und Bullinger (2000). Die alters- und geschlechtsgematchte gesunde Vergleichsstichprobe erhielt nur den SVF-KJ. Mädchen wandten hochsignifikant mehr die ungünstige Stressverarbeitungsstrategie gedankliche Weiterbeschäftigung als Jungen an. Darüber hinaus setzten Mädchen mit funktioneller Harninkontinenz hochsignifikant mehr die emotionsregulierende Bewältigungsstrategie “Ablenkung“ und tendenziell mehr die problemlösende Bewältigungsstrategie “Situationskontrolle“ zur Stressbewältigung ein als gesunde Mädchen. Korrelationsanalysen ergaben, dass eine hohe Lebensqualität mit günstigen Stress- und Krankheitsbewältigungsstrategien bei der klinischen Gruppe einher ging. Diese korrelativen Befunde unterstreichen die Bedeutung der Stress- und Krankheitsbewältigungskompetenz für die Gesundheit der Kinder mit funktioneller Harninkontinenz.
Abstract. Various results suggest that children and adolescents with functional urinary incontinence experience a considerable amount of illness-related stress. As coping abilities play an important moderating role in the impact of daily stressors on the course of disease, the aim of this study was to investigate differences in everyday coping styles in dependence of the group (functional urinary incontinence vs. healthy controls) and gender. Furthermore, the correlations between quality of life and coping styles were examined. In total, N = 78 children and adolescents with functional urinary incontinence (ages 8-13 years) were asked to complete a modified version of the German Coping Questionnaire for Children and Adolescents (SVF-KJ; Hampel, Petermann & Dickow, 2001) and the Kid-KINDLR (Ravens-Sieberer & Bullinger, 2000). Healthy controls were asked to fill out the SVF-KJ. Girls reported significantly more the maladaptive coping strategy rumination than boys. In addition, girls with functional urinary incontinence reported significantly more the emotion-focused strategy distraction and marginally more the problem-focused strategy situation control than healthy girls. In children and adolescents with functional urinary incontinence, high quality of life was positively correlated with adaptive everyday and illness-related coping strategies. In conclusion, these results emphasize the importance of effective coping with everyday and illness-related stressors for the health status of children with functional urinary incontinence.
