Abstract
Zusammenfassung. Im Kontext der klinischen Manifestationen, mit qualitativ zuverlässigen Labormethoden nachgewiesen und korrekt interpretiert, sind die ANA und ANCA ein unverzichtbares Werkzeug in der Differenzialdiagnose der entzündlich rheumatischen Erkrankungen. Die ANA und ANCA sind jedoch weder pathognomonische Marker für rheumatische Erkrankungen noch weisen sie einen zuverlässigen negativen Prädiktivwert auf. Zudem werden kommerziell produzierte Schnelltests wie der «Extrahierbare Nukleäre Antikörper-Screen» (ENA-Screen) oder der «Connective Tissue Disease Screen» (CTD-Screen) weitverbreitet angeboten. Leider tragen sie manchmal mehr zur allgemeinen Verunsicherung als zur Diagnosesicherung bei, wie das Fallbeispiel zeigt.
Abstract. In the context of clinical manifestations, if analysed with high quality laboratory methods and correctly interpreted, ANA and ANCA are an essential tool in the differential diagnosis of rheumatic diseases. Neither ANA nor ANCA, however, are pathognomonic markers of rheumatic diseases and they do not have a reliable negative predictive value. Commercially available screening tests such as ENA screen or CTD screen are offered widely. Unfortunately, the results from such tests may in some cases create more insecurity rather than confirming the diagnosis, as the presented case shows.
Résumé. Dans le cadre de manifestations cliniques, détectées avec des méthodes de laboratoire qualitativement fiables et correctement interprétées, les ANA et les ANCA sont un outil indispensable dans le diagnostic différentiel des maladies rhumatismales inflammatoires. Cependant, les ANA et les ANCA ne sont pas des marqueurs pathognomoniques des maladies rhumatismales et n’ont pas non plus de valeur prédictive négative fiable. En outre, des tests rapides produits commercialement, tels que le dépistage des anticorps nucléaires extractibles (ENA-Screen) ou le dépistage des maladies des tissus conjonctifs (CTD-Screen), sont largement disponibles. Malheureusement, elles contribuent parfois davantage à l’incertitude générale qu’à la fiabilité du diagnostic, comme le montre l’étude de cas.
