Vorsatzbildung und volitionale Unterstützung bei Gefäßpatienten
Effekte einer psychologischen Intervention auf das Ausmaß der körperlichen Aktivität
Abstract
Zusammenfassung. Die Teilhabe von Menschen mit einer peripheren arteriellen Verschlusskrankheit (pAVK) am Alltag ist aufgrund der krankheitsbedingten Beschwerden stark eingeschränkt. Sport- und Bewegungstherapie kann helfen, wenn das Gefäßsporttraining regelmäßig und dauerhaft durchgeführt wird. Diese beiden Voraussetzungen werden aber von einem Großteil der Patienten trotz hoher Intention nicht erfüllt. Um diese Intentions-Verhaltens-Lücke zu überwinden, wurde eine volitional ausgerichtete Intervention bei 90 Patienten in einem experimentellen Design hinsichtlich ihrer Wirksamkeit überprüft. In einer experimentellen Studie mit drei Messzeitpunkten wurden die Patienten der Experimentalgruppe in der Bildung von Ausführungs- und Bewältigungsplänen durch eine kurze psychologische Intervention unterstützt. Die Erhöhung der Handlungskontrolle wurde durch wöchentliche Erinnerungen an die aufgestellten Pläne über sechs Wochen angestrebt. Varianzanalytische Überprüfungen bestätigen die Hypothese, dass motivierte pAVK-Patienten von dieser Intervention profitieren. Sowohl die volitionale Kompetenz als auch die tatsächlich durchgeführte körperliche Aktivität unterscheiden sich signifikant von den Werten der Kontrollgruppe.
Abstract. People with peripheral arterial disease (PAD) have several limitations in everyday life, mostly because of their impaired walking ability. Prospective studies have demonstrated benefits of exercise on patients with PAD. A major problem is the deficient compliance of these patients. Despite the knowledge of the benefits of continuous exercise therapy and high intentions to act in this way, the majority of the concerned patients do not follow these references. The intention-behaviour gap should be solved by implying volitional aspects in addition to motivational factors. 90 participants with PAD participated in a longitudinal study with three measurement points. The intervention focussed on increasing action and coping planning by a short psychological intervention as well as action control. Analyses confirm the hypothetical assumptions. Highly motivated persons with PAD benefit from the volitional intervention. Volitional competence and the actual amount of physical activity have a significant higher value than in the control group, which was treated by the standard therapy for PAD.
