Welche psychosozialen Belastungen weisen Familien mit einem Kind mit einer Entwicklungsauffälligkeit auf und welche Unterstützungsangebote nutzen sie?
Ergebnisse einer längsschnittlichen Bevölkerungsstudie
Abstract
Zusammenfassung.Theoretischer Hintergrund: Früherkennung bei Entwicklungsauffälligkeiten (EA) ist wichtig, insbesondere wenn diese mit psychosozialer Belastung einhergehen. Fragestellung: Bisher ist wenig darüber bekannt, wie sich die Belastungssituation bei Familien mit Kind mit EA im Alter von 0 – 3 Jahren verändert und welche Unterstützungsangebote sie nutzen. Methode: 779 Eltern schätzten EA für ihr Kind in den Bereichen Kommunikation, Fein- und Grobmotorik, Problemlösekompetenz und Soziale Entwicklung ein und 765 Eltern füllten einen Fragebogen zu psychosozialen Belastungen (T1 und T2) aus. Ergebnisse: 30,6 % der Kinder zeigen eine EA. Die kumulierte Belastung ist bei Familien mit Kind mit EA erhöht. Im Bereich der Kommunikation, Feinmotorik und Problemlösekompetenz zeigt sich ein stärkerer Anstieg der Belastungskumulation, wenn keine Frühförderung genutzt wurde. Diskussion und Schlussfolgerung: Die elterliche Einschätzung beim frühen Erkennen einer EA sollte stärker in Betracht genommen werden.
Abstract.Theoretical background: Early detection of developmental delay (DD) is important, especially when associated with psychosocial stress. Additionally, support services should at best begin in early childhood, which is considered a sensitive phase of development. Objective: Little is known about the longitudinal associations between stressful situations in families with children aged 0 – 3 years with DD and the existing prevention offers for these families. This paper investigates (1) which psychosocial stress characteristics are longitudinally associated with DD and whether families with multiple stress characteristics are more likely to have children with DD later on; (2) the state of families with a child suffering from DD who use early intervention or targeted prevention offers. Method: 779 fathers and mothers who participated in the longitudinal survey “Kinder in Deutschland” – KiD 0 – 3 (“Children in Germany“) assessed DD with the German Version of the Ages and Stages Questionnaire in the areas of communication, fine and gross motor skills, problem-solving skills, and social development. 765 parents also completed a questionnaire on psychosocial stress (e. g., parenting stress, difficult temperament) in 2015 (T1) and two years later (T2). Results: 30.6 % of the children showed a DD in at least one developmental area. Longitudinal associations were found between difficulties in empathy, signs of depression and anxiety symptoms, and coparenting conflicts. In all developmental areas, families with a child with DD were more likely to report multiple stress characteristics (three or more). The parents of children with DD in gross motor skills were less likely to use family education services (at T2). Among families with a child with DD, the most frequently used service was early intervention. There is a greater increase in the accumulation of stress characteristics in the areas of communication, fine motor skills, and problem-solving skills when no early intervention was used or only at T2. In contrast to early intervention, at T1, the average number of stress characteristics was higher among users of targeted offers if a DD was present. However, at T2, there were no longer any significant differences in the average accumulation of stress between families with a child with and without a DD in any of the developmental domains. Discussion and conclusion: The use of early intervention at the earliest possible stage seems to have a positive impact on the families’ stress with a child with DD. Parental assessment in the early detection of DD is essential. Families with high stress levels may be supported in this task by the use of targeted prevention offers.
